$53,424 – $65,000 starting salary range.

Job Description

Tasks will include:

  • Clinician II will be discharging Chesapeake Residents from the State Mental

Hospitals, to include setting up aftercare and developing Discharge

Assistance Program (DAP) Plans, if appropriate. This position will work with

primarily Civil and Safety Net Patients.

  • The Clinician II will facilitate the discharge of consumers from the State

Mental Hospitals.

  • The Liaison will assist clients placed in the community until a case manager is

assigned or have their first psychiatric evaluation.

  • The Clinician II will work the Discharge Assistance Project (DAP) budget and

the Office of Census Reduction in order to facilitate consumer discharges

from the state mental health facility.

  • The Clinician II will open an electronic medical record and maintain

documentation on each consumer in a state mental health facility.

  • Establishes and maintains inter-agency relationships and other supportive

community services.

  • Maintains records according to existing standards and policies and to ensure

protection of client confidentiality and conducts quality reviews of all charts.

  • Prepares reports on clients for both inter-agency use and State Hospitals,

DBHDS and the Courts.

  • Provides individual transitional case management, residential and housing

support, crisis stabilization/intervention, and training and assistance with

activities of daily living.

  • Provides clinical consultation to Forensic staff.

Required Qualifications

Vocational/Educational Requirement: Requires a bachelor’s degree in social work, counseling, psychology, or a closely related field or other degree as approved by the Virginia Department of Health Professions.

Experience: In addition to satisfying the vocational/educational standard, this class requires a minimum of two years of related, full-time equivalent experience.

Special Certifications and Licenses: A valid driver’s license and a driving record that is in compliance with City Driving Standards is required. Qualified Mental Health Professional (QMHP) required; CPR and First Aid certifications within 3 months of hire.

Special Requirements: Employees may be expected to work hours in excess of their normally scheduled hours in response to short-term department needs and/or City-wide emergencies. Emergency operations support work and work locations may be outside of normal job duties.

Preferred Qualifications

  • Four Years experience in mental health and substance abuse counseling, to

include case management experience.

  • Must be a Qualified Mental Health Professional.
  • State hospital Discharge Planning experience preferred.
