Registered Nurse (RN) – Case Manager Hiring Now!

Full-Time | 8:30 AM – 5:30 PM

We are seeking a compassionate and dedicated Registered Nurse (RN) Case Manager to join a dynamic healthcare team. This role is ideal for a nurse who thrives in a collaborative, patient-centered environment and is passionate about delivering high-quality primary care. You’ll work closely with a family physician to coordinate care, conduct office visits, and provide patient education.

Key Responsibilities:

• Collaborate directly with a family physician to support primary care delivery
• Conduct in-person office visits and follow-up outreach as directed
• Educate patients on treatment plans both in person and electronically
• Assist with a variety of clinical and administrative tasks
• Travel to multiple locations within Santa Barbara County as needed

Qualifications:

• Active RN license in California
• Minimum of 1 year of RN experience (hospital or outpatient setting)
• Strong communication and organizational skills
• Willingness to travel to various locations
• Bilingual (Spanish/English) preferred, but not required

Pay and Benefits

The pay range for this position is $25.00 - $25.00/hr.
