Social Worker, Case Manager - U.S. Air Force Academy

SUMMARY

Perform healthcare coordination and case management activities in accordance with Medical Treatment Facility (MTF) instructions. May be required to function in a specialty mental health clinic or in a patient centered medical home (PCMH) location as a behavioral health consultant (BHC).

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Maintain accurate and current notes in the Mental Health records and electronic medical records of all patients seen, as appropriate, and produce reports of evaluation and/or treatment, as required.
  • Responsible for and applies psychological procedures and techniques, including interviewing and behavioral assessment in the evaluation and diagnosis of psychological and neuropsychological disorders.
  • Contact referred patients to assess treatment adherence.
  • Use validated symptom inventories to assess current symptom severity.
  • Assess barriers to treatment adherence and help patient problem-solve solutions to barriers.
  • Provide patients with education regarding their mental health condition and treatment regimen.
  • Routinely staff difficult cases with the referring provider, behavioral health consultant and/or psychiatry consultant.
  • Perform case management functions.
  • Maintain regular and punctual attendance.
  • Perform other duties as assigned.

SUPERVISORY RESPONSIBILITIES

None.

MINIMUM QUALIFICATIONS

  • Master of Social Work (M.S.W.) degree from a graduate school of social work fully accredited by the Council on Social Work Education (CSWE) required.
  • Maintain a current, unrestricted clinical license to independently practice social work in any one of the 50 states, the District of Columbia, Puerto Rico, or the U.S. Virgin Islands.
  • Have a minimum of two (2) years within the past five (5) years in the independent practice of clinical social work in a mental health setting working with an adult population, or with children and teens if specifically working with that population.
  • Must have experience in brief behavioral interventions.
  • Must follow site required health screening and immunization requirements.
  • Must obtain and maintain any necessary security access and/or background checks.

DESIRED QUALIFICATIONS & SKILLS

  • Experience working in a government setting such a DOD or Department of Veterans Affairs (VA) MTF is preferred.
  • Demonstrates knowledge, skills, and ability to use basic office automation and information management equipment and programs (i.e., word processing, database management, email, etc.).
  • Demonstrate a working knowledge of professional standards and ethics regarding the delivery of clinical social work services.
  • Excellent oral communication skills for patient education and telephone management of patients.
  • Demonstrates sufficient initiative, interpersonal relationship skills, and social sensitivity to effectively relate to a diverse range of patients and healthcare staff.
  • Ability to effectively read, write and speak English in order to communicate with all patients and other healthcare providers.
  • Ability to effectively communicate and collaborate with a diverse group of people for the purpose of informing the healthcare team of plans/actions, for teaching/education to benefit the patient/family and organization.
  • Excellent written communication skills to perform accurate documentation, both written and electronic, of all activity in accordance with requirements.

WORK SCHEDULE

Full-time. May be required to work additional hours as needed to complete assignment or project.

TRAVEL

All Market contractors are subject to working at a duty location at any of the MTFs or associated sub clinics within the Colorado Military Health System (CMHS).

WORK ENVIRONMENT

Work is performed primarily indoors at a well-lit military treatment facility such as a clinic, outpatient facility or hospital. The environment is normally air conditioned, but conditions may change dependent upon circumstances. Work may need to be performed in a fast-paced environment requiring quick thinking and rapid judgements. Employee will be exposed to a wide variety of clients in differing functions, personalities and abilities. Exposure to bloodborne pathogens and bodily fluids, latex, radiation or other contaminants and safety issues. May be exposed to infection or diseases from contact with patients. Work is highly social with patients and can be stressful with a high emotional element. Decisions made may substantially impact patients and their families.

PHYSICAL DEMANDS

Position consists of sitting for long periods of time, using hands/fingers, handling, reaching with hands and arms, speaking, or translating. Periods of walking, standing, bending, kneeling, stooping, crouching, and lifting boxes weighing up to 30 pounds. Moderate to extended periods of office work with heavy periods of computer usage. COMPENSATION & BENEFITS PACKAGE

  • Pay range: $29.00 to $30.17 per hour.
  • H&W: $4.93
