Pay Range: $16.85 - $23.14

Position Objective:

The Patient Access Clinical Triage Rep is responsible for providing triage (sorting/prioritizing patients) telephone service to ensure prompt identification of patient with high-risk conditions, assessing the need for clinical referral, providing education, and carrying out such other Medical Assistant duties as PASS may need. The Patient Access Clinical Triage Rep is a key member of the care team working to provide quality comprehensive services to patients while collaboratively with staff to provide team-based coordinated care. The Patient Access Clinical Triage Rep will demonstrate the values consistent with Patient Access and Support Service’s mission.

Essential Job Duties:

1. Accurately documents symptoms/complaints, clinical assessment, advice provided and patient/caller response.
2. Utilize standing orders, triage protocols and algorithms appropriately
3. Manage prescription refills per protocol
4. Provide patient education: e.g. Chronic disease, preventative and family
5. Participates in Performance Improvement activities as assigned
6. Documents in electronic health record (EHR), triage assessment, plan, implementation, and evaluation and follow-up
7. Obtains appropriate input and direction from the provider as needed to determine best clinical course of action and recommendations for specific patient health care needs.
8. Coordinates and schedules same day appointments based on clinical need
9. Evaluates and assesses patient’s health care needs based on patient’s signs and symptoms utilizing established triage protocols and guidelines.
10. Assures that pertinent medical information is forwarded to proper departments as needed for continuity of patient care.

Educational/Experience Requirements:

• Three to five years working in an Ambulatory Care or related field. Demonstrated experience in staff development theory and/or principles of adult learning
• Excellent customer service skills required. Must demonstrate excellent professional and courteous interaction with patients, co-workers, physicians, and other health care professionals.
• Strong technical skills, preferably using an Electronic Health Record software
• Excellent verbal and written communication skills

Required License/Certifications:

• Registered or Certified Medical Assistant
• CPR Certification

Working Conditions, Equipment, Physical Demands:

There is a reasonable expectation that employees in this position will not be exposed to blood-borne pathogens.

Physical Demands:

Light work. Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects. If the use of arm and/or leg controls requires exertion of forces greater than that for sedentary work and the worker sits most of the time, the job is rated for light work.
