JOB SUMMARY
The Survivorship Program Nurse Coordinator provides resources and education for patients at diagnosis, through cancer treatment and beyond. The Survivorship Coordinator utilizes the nursing process to assess, plan, implement, and evaluate comprehensive care to persons with cancer and their families. The Survivorship Program Coordinator interacts with physicians, pharmacy, Clinic personnel, and community agencies, home care nurses, and outside physician offices to provide optimal care to the individual diagnosed with cancer and their family. They must remain calm in stressful situations and represent the Department of Oncology/Hematology in a positive and professional manner.
JOB QUALIFICATIONS
EDUCATION
For positions requiring education beyond a high school diploma or equivalent, educational qualifications must be from an institution whose accreditation is recognized by the Council for Higher Education and Accreditation.
Minimum Required: Graduate of an accredited school of Nursing.
Preferred/Optional: None
EXPERIENCE
Minimum Required: Theoretical knowledge and technical skills in teaching and/or patient education. Prior experience with program development and formal community and family caregiver support modalities and local resources.
Preferred/Optional: Minimum two years’ nursing experience. Oncology nurse experience.
