PURPOSE OF POSITION
To Provide Pharmaceutical Services.
AREAS OF RESPONSIBILITIES
OPD Pharmacy.
SUMMARY OF DUTIES AND RESPONSIBILITIES
I. FUNCTION:
Under the direct supervision of the Director of Pharmacy or his/her designee, supervises the Outpatient Pharmacy operation, including inventory control and staffing. Ensures patient safety by accurately and appropriately dispensing and preparing medications for outpatient dispensing.
II. POSITION SPECIFIC COMPETENCIES:
1. Demonstrates skills needed to Supervise the Outpatient Pharmacy Operation
2. Verifies medications and dispense medications to outpatients as deemed necessary
3. Exhibits knowledge and skills to conduct patient counseling.
4. Interacts with other staff members, patients and visitors, serves on interdisciplinary teams
5. Manages the inventory and complies with the required regulations pertaining to the practice of pharmacy.
III. MAJOR DUTIES:
1. Responsible for filling out-patient prescriptions according to rules and regulations of State Board of Pharmacy.
2. Participates in the new departmental patient care evaluation and quality assurance programs.
3. Responsible for recording and monitoring and transferring information to patient profiles.
4. Periodically checks medications for valid expiration dates.
5. Participates in continuing education, in-service education, and Clinical Programs.
6. Coordinates the dissemination of drug information to Providers, Nurses and Patients.
7. Maintains records of all controlled drugs dispensed to Patients.
8. Maintains complete patient profiles; checks for drug interactions, appropriate dosing of medication, allergies, and diagnosis, patient’s weight, drug-food interactions, etc.
9. Supervises the pharmacists and technicians in all aspects of Pharmaceutical Functions
10. Supervise and controls regular medications and narcotic inventory
11. Conducts Nursing and Child Health Clinic Inspections whenever needed
12. Participates in the following:
a. Screens and properly fills and dispenses prescriptions for outpatients.
b. Answers telephone requests from physicians or professional staff for drug information and resolves problems.
c. Departmental meetings and interdisciplinary meetings
For Level I:
1. Possession of a valid NY State license to practice pharmacy.
For Level II: In addition to meeting the qualification requirements of the lower level:
1. Two years of post licensure experience; or
2. Completion of a post graduate specialty residency or fellowship program; or
3. Current Pharmacy Board Certification.
For Level III: In addition to meeting the qualification requirements of the lower levels, an additional:
1. One year of post licensure supervisory experience or specialty clinical experience.
