As a Behavioral Health Case Manager II, you will work under clinical supervision to improve the functioning of clients/patients with diverse needs. Your responsibilities will include developing treatment plans, coordinating community resources, providing direct services, and monitoring client progress. You will also assist clients in navigating financial entitlements, learning essential life skills, and accessing vocational rehabilitation services.

Key Responsibilities:

  • Collaborate with the clinical team to develop individualized treatment plans that address the unique strengths, needs, abilities, and preferences of each client.
  • Connect clients with essential community resources, including housing, social services, and substance abuse treatment, to support their overall well-being.
  • Provide direct support to adults, children, and families, empowering them to achieve their treatment goals and improve their quality of life.
  • Advocate for clients' rights and ensure they have access to the services and support they need to thrive.
  • Monitor and assess clients' progress, providing ongoing feedback to the treatment team and making adjustments to the plan as needed.
  • Assist clients in developing essential life skills, such as personal health management, self-advocacy, and independent living skills.
  • Develop supports for clients who are employed or involved in employment programs and teach consumers how to maintain employment.

Education, Experience, and/or Credential Qualifications:

  • Bachelor's degree in social service, education, behavioral health, or other health-related field from an accredited college is required.
  • Documented experience in the area of case management services is preferred.
  • Must demonstrate current certification as a Certified Behavioral Health Case Manager (BHCM) II with the Oklahoma Department of Mental Health and Substance Abuse Services.

Additional Qualifications:

  • Current driver’s license, acceptable driving record, and current auto insurance are required.

Physical Requirements:

  • ADA Consideration - Sedentary work: Exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently to lift, carry, push, or pull, or otherwise move objects, including the human body.
  • Repetitive movements of hands, fingers, and arms for typing and/or writing during work shift.
  • Involves sitting most of the time but may involve walking or standing for brief periods of time.
