Day Shift - 10 Hours (United States of America)

The Clinical Nurse Navigator serves as a patient navigator throughout the continuum of care. Collaborates with multidisciplinary team members in the acute, post-acute and outpatient settings to identify patient needs such as home health care, physical therapy and community services Functions as a liaison for management staff, administration, physicians, community organization, and other customers. Maintain hospital program certification

• Excellent interpersonal and customer service skills.
• Strong problem solving skills with a flexible and creative approach.
• Ability to work independently.
• Excellent oral and written communication skills; able to communicate professionally and effectively with all member of the healthcare team as well as in simple, concise and soothing language to patients and their families.
• Uses appropriate telephone etiquette when performing telephonic duties.
• Ability to operate a computer and/or a tablet PC.
• Capability to handle stressful situations

Certifications

NYS RN License preferred

NCCP Certification preferred

Education Requirements

BA/BS required

RN License

Computer Skills

MS Office

Salary Range: $55/hr - $57/hr
