We’re looking for a proactive, detail-savvy Annual Wellness Visit Navigator to help drive the success of our AWV program and empower Medicare beneficiaries with the preventive care they deserve.

???? What You’ll Be Doing:

???? Program Management

  • Own the AWV scheduling and follow-up process
  • Track AWV completion rates and optimize workflows
  • Ensure compliance with Medicare timelines and regulations
  • Implement strategies to boost participation and outcomes

???? Patient Engagement

  • Educate patients on the benefits of Annual Wellness Visits
  • Support patients through the Health Risk Assessment process
  • Help patients navigate screenings, assessments, and portal usage
  • Be a friendly, reliable point of contact from start to finish

???? Clinical & Administrative Support

  • Collect and document key wellness visit data: HRA, medical history, vitals, cognitive function
  • Assist in developing personalized preventive care plans
  • Ensure accurate documentation for billing, compliance, and quality reporting
  • Coordinate follow-ups, referrals, and transitions of care

???? Care Coordination

  • Collaborate with providers, care teams, and community resources
  • Ensure a smooth, connected care experience for every patient
  • Help close care gaps and reduce barriers to access

✅ What We’re Looking For:

Education & Certification

  • Certified Medical Assistant or equivalent experience
  • AWV Certificate Course (9 hours specialized training) to be completed within 6 months of hire
  • Bonus points for additional care coordination/patient navigation credentials

Experience

  • 2+ years in a healthcare setting
  • Familiarity with Medicare AWV guidelines and preventive care standards
  • Comfort using EHRs and care management platforms
