Job Title: RN Case Manager

Job Summary:

We are seeking a dedicated and compassionate RN Case Manager to coordinate case management activities in compliance with company policies and proper documentation. This role involves managing complex and acute cases to ensure members receive high-quality, cost-effective healthcare services that lead to optimal health outcomes.

Responsibilities:

  • Conduct intake assessments with patients and families via telephonic case management.
  • Ensure families feel adequately supported and equipped.
  • Develop individualized treatment plans considering physical, financial, and other constraints.
  • Adjust treatment protocols as needed to ensure effective patient care.
  • Suggest and coordinate appropriate follow-up services to promote wellness.
  • Ensure patients have transportation to and from their homes and healthcare facilities.
  • Meet with healthcare teams to discuss patient progress and determine appropriate care.
  • Provide cost-saving resources, including alternative funding programs, manufacturer assistance, and grants.
  • Verify insurance coverage and financial options for patient treatments.
  • Develop innovative solutions or suggest referrals for patients who face financial barriers to care.
  • Maintain confidentiality in accordance with the Health Insurance Portability and Accountability Act (HIPAA).

Pay and Benefits

The pay range for this position is $36.06 - $38.46/hr.
