Job Summary
Make a meaningful impact in your community—join Acentra Health as a Clinical Evaluator (PASRR) in Port Angeles, Washington. This full-time hybrid role offers the best of both worlds: the flexibility of working remotely from your home office and the fulfillment of conducting in-person assessments in nursing homes, hospitals, and community-based settings throughout Port Angeles, Washington and the surrounding areas. Travel expenses, including mileage and parking, are fully reimbursed.
As a Clinical Evaluator, you’ll conduct face-to-face PASRR assessments to help determine appropriate placement and next levels of care for individuals in need and the priority populations .Your work will play a vital role in care planning, placement decisions, and facilitating next-level care, making a meaingful impact in the quality of lives of those within our community.

Compensation
The compensation for this role is $85.000.00 to $93,600.00 base salary
Based on our compensation program, an applicant’s position placement in the pay range will depend on various considerations, such as years of applicable experience and skill level.
