The Quality Assurance Nurse has accountabilities for the monitoring, implementation and evaluation of process and programs to meet regulatory, accreditation and standards of practice.

Essential Job Functions/Responsibilities

1. Responsible for the implementation and monitoring of the organization’s quality assessment performance improvement (QAPI) program.

  • Monitors and measures activities related to QAPI as assigned.
  • Implements quality improvement activities utilizing continuous quality improvement principles and methodologies.
  • Implements established schedule for clinical record review and data collection.
  • Compiles, trends and reports quality data in the following areas: patient care, safety, risk management, infection control, outcomes, and customer satisfaction.
  • Works collaboratively with nursing department to assure documentation is complete and consistent with care and reflects regulatory/legal requirements.

2. Responsible for implementing processes to monitor and evaluate safety, risk management and infection control programs.

  • Implements monitoring activities for safety, risk management, and infection control.
  • Assists with mandatory in-service programs on OSHA regulations for safety and infection control as requested.
  • Acts as resource to staff in identifying safety and risk management issues for patients and staff.
  • Reviews all incident reports for completeness, risk, standard of care determination and appropriate follow-up. Trends data and makes recommendations related to trends identified.
  • Maintains current knowledge related to home care safety and infection control standards.

3. Monitor organization compliance with regulatory and accreditation standards.

  • Maintains current knowledge related to Medicare Conditions of Participation and the Community Health Accreditation Program’s (CHAP) standards to ensure that the organization is compliant with state, federal and accreditation guidelines.
  • Participates in review of policies and procedures and makes recommendations for policy changes to the Executive Director of Quality.
  • Acts as a resource to the clinical team regarding standards, policies, and regulations.

4. Acts as a clinical resource to staff.

  • Maintains updated nursing knowledge and skills.
  • Assists with patient visits as needed.

5. Provides assistance to Care Transition Coordinators in meeting community education needs.

  • Maintain positive relationships with referral sources.
  • Maintain positive image in the community.
  • Acts as representative of the organization at community meetings, civic and professional organizations as needed.

6. Promote self-growth and collegial relationships with others in the home care industry.

  • Maintains professional and technical knowledge by attending educational workshops, reviewing professional publications, establishing personal and professional networks and participates in professional organizations or societies.

  • Registered nurse with current licensure to practice professional nursing in the State.
  • Bachelor’s degree in Nursing from an accredited program by the National League
  • for Nursing is preferred.
  • CHPN certification or able to obtain.
  • Three (3) to five (5) year's clinical nursing experience. (Hospice nursing is preferred).
  • Experience with performance improvement, quality assessment, and utilization management.
  • Is self-directed and able to work with minimal supervision.
  • Has excellent coordination and communication skills.

The starting salary range for this role is $77,400-$96,000 annual base salary. An employee’s actual annual salary will be based on but not limited to: geographic location, relevant education, applicable certifications, depth of experience, and special skillsets. The total compensation package for this position may also include the opportunity to participate in employee benefits including our medical/dental/vision/pet insurance, disability and life insurance, paid time off, and 401(k) retirement plans, subject to applicable plan terms, and/or eligibility for other compensation.
