Summary

Serves as the first contact for our customers and staff providing all clerical functions necessary to prepare and maintain a complete, accurate medical record. This includes; identifying and labeling any documents that must be scanned into the medical record and reviewing charts, assigning any deficiencies to assure that all providers complete all necessary components of that chart per Centers for Medicare and Medicaid Services, The Joint Commission and corporate rules and regulations. Good communication skills are a must as analysts will work closely with our patients, insurance companies, employers, other staff members, etc. in coordinating the release of health information in an accurate and timely manner in accordance to both state and federal laws.

Responsibilities

(25%)* Performs all clerical functions necessary to organize, maintain, and use patient medical records. Will train in one or more specialty clerical functions. Clerical assignments may vary from day-to-day depending on the needs of the department.

(25%)* Receives telephone calls, faxes, and visitors to Health Information Services and assists record/portal users.

(25%)* Works with physician/staff and monitors incomplete patient records to ensure that the record completion process is competed in a timely and efficient manner. Notifies physicians about their incomplete records.

(25%)* Coordinates the release of medical information to all requesters, including patients, physicians, other health care providers, insurance companies, and lawyers, in accordance with medical center and federal and state laws.

OTHER DUTIES AND RESPONSIBILITIES:

Assembles medical records into standard record format and maintains record order on a continuous basis.

Reviews discharged patient records to determine whether they are complete and assigns deficiencies to the appropriate physician/staff for completion when they are not.

Locates and provides individual patient records and sets of records on an as-needed basis.

Files medical records following their use and/or files individual reports into patient records on receipt.

Expands job-related knowledge and skills to improve performance and adjust to change.

Uses interpersonal skills effectively to build and maintain cooperative working relationships.

Performs other related miscellaneous duties.

Required Education

High school diploma or GED is required

EXPERIENCE, TRAINING AND SKILLS:

Prior Health Information Services experience preferred.

Successful completion of the Office Worker’s Test with a minimum score of 70%.

Successful completion of the Typing Test with a minimum speed of 40 WPM.

Must be able to coordinate efforts with coworkers to maximize productivity and quality of services.

Participates in Continuous Quality Improvement as required by the Health System and the job description.

Understands and is accountable for Health System's customer service standards.

Strong customer service orientation.

Good communication skills.

Basic computer (keyboard) knowledge.

PHYSICAL/MENTAL REQUIREMENTS AND TYPICAL WORKING CONDITIONS:

Exposure to stressful situations, including those involving public contact, as well as, trauma, grief and death.

Able to wear personal protective equipment that includes latex materials or appropriate substitute if required for your position.

Is able to move freely about facility with or without an assisted device and must be able to perform the functions of the job as outlined in the job description.

Overall vision and hearing is necessary with or without assisted device(s).

Frequently required to sit/stand/walk for long periods of time. May require frequent postural changes such as stooping, kneeling or crouching.

Some exposure to blood borne pathogens and other potentially infectious material. Must follow MyMichigan Health bloodborne pathogen and TB testing as required.

Ability to handle multiple tasks, get along with others, work independently, regular and predictable attendance and ability to stay awake.

Overall dexterity is required including handling, reaching, grasping, fingering and feeling. May require repetition of these movements on a regular to frequent basis.

Physical Demand Level: Sedentary. Must be able to occasionally (0-33% of the workday) lift or carry 0-10 lbs.
