Job Summary:

The Hospice Chaplain seeks to provide on a non-sectarian basis access to spiritual care and resources for all Hospice patients and their families upon request, ongoing and at the death of the patient. The chaplain works closely with local churches and serves as a liaison to community chaplains in providing spiritual support to patients and family members. The Hospice chaplain provides spiritual counseling based on the initial and ongoing assessment of spiritual needs of the patient/family. Works closely with IDG in providing consultation support as needed. Chaplain reports to the Hospice Director. Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served; also demonstrates knowledge of the principles of growth and development and possesses the ability to assess data reflective of the patient’s status and interprets the appropriate information needed to identify each patient’s requirements relative to his/her age specific needs, and to provide the care needed. Participates in in-service programs and CEU’s as applicable. Prepares documentation of patient care in the home at the time of visit as applicable and submits documentation in a timely manner according to policy. Notifies health care team of any changes and assist patient and family in meeting needs. Performs other job duties as assigned.

Performance Expectation:

  • Demonstrates ability to utilize computer and EMR applications for documentations of assessments and for performing activities related to Bereavement program.
  • Demonstrate ability to enter all patients charting in EMR documentation meeting state and federal regulations.

Qualifications:

Education/Skills

Undergraduate degree in religion required. Completion of Theology degree

preferred. Four units of clinical Pastoral Education required; additional two advanced units preferred.

Work Experience:

Two years in pastoral or chaplaincy position required.

Certification/Licensure-DUE UPON HIRE

  • Certification by Ecclesiastical body to which Chaplin belongs. Fellow in the College of Chaplins, preferred.
  • Valid Driver's License and proof of automobile insurance required.

Additional Certification/Licensure - Obtained based on required timeframe below

  • Basic Life Support
