Systematically evaluates the strengths, needs, preferences, and challenges of infants and children, in collaboration with the family, as part of a NJ Early Intervention Team. Collaborates with family and other Individual Family Service Plan Team Members to develop an Individualized Family Service Plan (IFSP) which incorporates family priorities/concerns, clinical observation, and evaluation/assessment data to develop child and family outcomes. Implements evidenced-based instructional practices in the child’s natural environment. Provides education, resources, and coaching to clients, family, caregivers, and health care professionals to promote and maintain health and well-being.
QUALIFICATIONS: Bachelor of Science Degree in Physical Therapy required. 1 year of documented professional experience with individuals from birth to five years of age who have developmental delays and disabilities. Licensed Physical Therapist, State of New Jersey.
