LPN - ER Care Continuum (PH)

12p-9p, Monday - Thursday

I. Position Summary:

Primarily responsible for coordinating the continuum of care for all patients referred to the ED. Will facilitate radio and telephone communication for incoming ED patients. Obtain and update all documentation in the electronic medical record to include but not limited to patient history, diagnosis, transfer facility, call back numbers, etc…. Consult with ED physician and charge nurse to evaluate severity of illness and intensity of services to be provided and to determine appropriateness of ED acceptance of transfers. Acts as a liaison with ED Physician and consulting physicians. Facilitate and/or assist with admissions by collaborating with transfer facility, charge nurse, house supervisor or bed coordinator. Will obtain verbal (telephone) orders as necessary. Ensures a positive and seamless interaction with the ED and other disciplines/units/facilities to ensure optimum patient care. Follow up on lab and radiology results and consults with physician or designee in order provide full follow up for proper treatment (prescriptions and follow up calls). Will assist with pharmacy call backs and other ED calls that may require chart review. Assist patients and family members in obtaining referrals, communicating with physicians for follow up as requested. Coordinates plan of care with the patient, their family and the interdisciplinary team. Ensures appropriate resources to all internal/external customers. Responsible for carrying out the mission, vision, and quality commitment of Houston Healthcare.

II. Qualifications:

A. Education & Training: Graduate from an accredited School of Practical Nursing.

B. Experience: Four (4) years nursing experience.

C. Required Certification/Registration/Licensure: Current US state LPN license with authorization to practice in the state of GA. Current AHA/ARC BLS certification. AHA ACLS/ARC ALS and AHA/ARC PALS required within nine months of hire. CPT training within 9 months of hire. Must complete and maintain required stroke education/training as mandated in the “HHC Primary Stroke Center / Acute Stroke Ready Designation” policy.

D. Knowledge, Skills & Abilities: Excellent computer and data entry skills. Excellent communication (verbal and written) and interpersonal skills. Ability to work competently without constant instruction from others. Ability to complete the task accurately every time. Ability to maintain accuracy even in stressful and emergency situations. Ability to perform each essential duty satisfactorily. Knowledge of EMS roles and protocols as well as trauma criteria and deferring criteria. Knowledge of electronic medical record documentation systems. Must be able to utilize personal protective equipment such as gloves, gown, mask, etc.
