  • Hours: 40hrs

  • Location: Primary Care – Fairhaven, MA
  • Travel: Driving on average 3 or more times per week required

We are searching for a TL-Chronic Care Mngt-LPN to perform a variety of duties involving the coordination of clinical support, personnel productivity monitoring and a reduced patient panel. Provides guidance and support to clinical staff. Implement and monitor clinical workflows. Collaborate with Professional Development staff to onboard, educate, assess competency of, and monitor clinical staff. In collaboration with practice leadership, ensure clinical practice of care to promote safe and high-quality patient care. Duties require direct supervisory responsibility for all assigned personnel and activities. Provide technical supervision, working in collaboration with the program manager to monitor attendance and productivity. Work with practice management and providers to ensure adherence of chronic care management with CMS requirements, office policies and procedures

Education and/or Experience Requirements:

  • Minimum of five years nursing or relevant care/case management experience required.
  • Three years of medical office experience preferred.
  • Supervisory experience preferred.
  • Knowledge of population health management is strongly preferred.
  • Must possess and demonstrate highly effective interpersonal, communication and leadership skills and be able to manage multiple priorities.
  • Previous leadership training is preferred.
  • Requires computer skills including but not limited to Microsoft Office and Electronic Health Record (EHR) experience.
