Position Summary:
The Physical Therapist Assistant is responsible for assuring that the PNHS values of Head + Heart, Together, are achieved through daily departmental operations. This includes, but is not limited to, direct patient care, professional growth and development, compliance with regulatory standards and participation in process improvements to advance patient care outcomes and satisfaction.
The Physical Therapist Assistant will provide patient therapy treatment and teaching to patients and families in their home in accordance with the plan of care established by the Physical Therapist. The Physical Therapist Assistant will communicate in a timely, clear, and professional manner with staff, patients, patient family members, physician, and other internal/ external customers. The staff member will demonstrate skills in listening, conflict management, system problem solving, decision making and process improvement. Physical Therapist Assistants will practice in compliance with professional standards of practice and professional code of ethics. The assistant supports the following community care processes: admission, discharge, referrals to other disciplines, billing, and care delivery.

M-F 8am - 4:30pm
