Planned Parenthood of Northern New England is seeking a Remote Care Team Registered Nurse (RN) to fill a full time, 37.5 hour per week position is based within Maine, Vermont or New Hampshire. As an integral part of health care delivery at PPNNE, the Remote Care Team (RCT) provides both Telehealth clinical care to patients, as well as Centralized Clinical Support to both patients and health center staff at our 16 health centers regarding lab work follow up, patient communications, referral care coordination, and health information management Affiliate-wide. The Remote Care Team Nurse may serve as the first point of contact for patients via remote support services, ensuring outstanding customer service and patient satisfaction by collaborating with the team to provide high-quality patient centered care, while also attending to daily administrative centralized support duties Affiliate-wide.

CHARACTERISTIC RESPONSIBILITIES:

  • Demonstrate approach to sexual and reproductive health care consistent with Planned Parenthood’s philosophy, service standards and fundamental concepts of reproductive justice.
  • Demonstrates a commitment to recognizing and mitigating unconscious bias in the provision of sexual and reproductive health care.
  • Provide excellent, patient-centered care in collaboration with PPNNE colleagues including licensed and non-licensed staff
  • Provide health care that is culturally and linguistically appropriate to PPNNE patient populations
  • Counsel/educate patients regarding all services offered by PPNNE including general reproductive health care; all FDA-approved methods of birth control, including emergency contraception; pregnancy options; and other services related to physical and emotional health and wellbeing
  • Adhere to all reporting requirements and state laws relevant to the abuse of minors and relevant to infectious disease reporting
  • Utilize multiple digital applications simultaneously including; Electronic Medical Record (EMR), Microsoft 365 applications (Excel, Teams, Outlook etc), RingCentral phones/faxing; and other technologies as needed, or willingness to learn.

Perform Remote Care Team Nurse duties, including:

  • Assist RCT Manager and Assistant Manager with assigned responsibilities, primarily related to follow up of lab results, incoming patient medical questions, and care coordination in collaboration with members of the Remote Care Team
  • Per standing orders, or in consultation with RCT Clinical Director and Clinical leadership team, provide patients with information regarding lab test results:
  • Notify patients of results, follow up care plans, and send reminders for care
  • Complete communications within appropriate timeframes per Medical Standards and Guidelines (MS&G)
  • Document clearly all outgoing and incoming communications and update EHR to reflect appropriate alerts and next stages in follow up care coordination as required per MS&G
  • Triage and respond to incoming patient communications (via telephone and electronic communications) per MS&G, standing orders, and/or in consultation with RCT Clinical Director
  • Per standing orders, fulfill prescription treatments based on lab result findings and extension refill requests
  • Support Affiliate APCs with developing centralized process and completion of Prior Authorization requests
  • Schedule patients for appointments based on medication, symptom or follow up needs
  • Act as Affiliate care coordinator for high urgency lab results and clinical cases in collaboration with health center staff, Remote Care Team Associates, on-call services, and lab vendors as needed
  • Work closely with and serve as clinical resource to Remote Care Team Associates
  • In collaboration with Assistant Manager and RCT Clinical director, prepare various clinical and administrative records and reports
  • Assist with regular CQI initiatives and associated required audits in collaboration with, RCT Clinical Director, Regional Clinical Directors and Director of Risk-Quality Management
  • Perform other relevant duties as assigned

QUALIFICATIONS:

  • RN, with associate degree in nursing (bachelor's degree preferred), plus 1-3 years of relevant clinical experience, or an equivalent combination of education and experience from which comparable knowledge and skills are acquired
  • Compact RN license in Vermont, New Hampshire and Maine
  • Ability to work completely remotely and collaborate effectively with remote team members across 3 states
  • Excellent customer service skills and ability to discuss sensitive topics using trauma informed approach to care
  • Ability to work evenings
  • Basic clinical knowledge related to reproductive health care or willingness to learn
  • Experience with Electronic Health Record (Epic EHR system preferred)
  • Experience with telephone and EHR portal triage
  • Ability to navigate multiple digital applications at once including EHR, Microsoft 365 (Excel, Word, Teams, SharePoint), Outlook, RingCentral phones/faxing, and other technologies as needed, or willingness to learn
  • Ability to sit or stand for up to 6-7.5 hours per day
  • Ability to operate all standard office equipment
  • Staff providing care remotely must meet the requirements in Policy 340: Flexible Working Arrangements and Telecommuting (see PPNNE Personnel Policies)

Compensation & Benefits

  • Compensation for RN positions is commensurate with the number of years since licensure, with the range of pay between 34.86/hr and $50/hr. Access to benefits is available and includes health, dental and vision coverage, paid time off and a retirement fund.
