This position is responsible for performing and or/ coordinating appropriate utilization of resources accomplished through efforts that are cost-effective, safe, and clinically appropriate.

Coordinates care and progression status, and facilitates movement to the most appropriate level of care. Proactively evaluates the patient needs and provides discharge support if indicated. Integrates the role with other departments as well as working closely with the ED and Admitting physicians in order to expedite the correct level of care.

This will include implementation of procedures consistent with the goals and objectives of the Sinai Health System Utilization Review Plan.

MINIMUM Education:

  • Associate degree in Nursing required, Bachelor’s degree in Nursing preferred

MINIMUM WORK EXPERIENCE:

  • Three years of healthcare case management experience

KNOWLEDGE & SKILLS:

  • Ability to create and analyze data, perform gap analysis. 
  • Proficient in Excel and other Microsoft Office products
  • Outstanding oral and written communication skills
  • Ability to work independently in accordance with policy and procedure.
  • Ability to thrive in an unstructured environment

REQUIRED LICENSES, Certificates, Registrations:

Registered nurse in the state of Illinois
