JOB SUMMARY:

Applies nursing and scientific principles to thoroughly assess patients on a continuous basis. Assesses patients' potential for wellness and response to various treatment strategies. Review charts and incorporate findings of other disciplines to facilitate a plan of care and enhance outcome. Utilizes recognized standards to evaluate the effectiveness of nursing care. Re-evaluates and modifies plan of care to consistently meet the appropriate level of care. Documents relevant patient assessment and care information every shift.

Care of Patient:

Promotes and implements the concept of patient and family-centered care. Notifies the physician of laboratory results, radiological findings, and significant alteration in patient status. Manages the care of the pre- and post-operative/procedural patient according to hospital policies and standards of care. Responds to patients' needs in a timely manner. Accurately implements the medical plan of care according to identified treatment priorities. Delegates appropriately and ensure completion of tasks. Demonstrates knowledge of pain assessment and implements appropriate strategies for pain management. Applies restraints in clinically justified situations, monitors patients, and documents according to policy and procedure. Understands and incorporates cultural diversity into all aspects of patient care. Accompanies the patient on clinically indicated transport. Recognizes and responds to patient emergencies. Verifies accuracy of transcription of physician orders. Participates in discharge planning. Promotes a compassionate, supportive, and therapeutic environment for the patients, families, and co-workers. Performs other duties as directed.

Education:

Assesses patients' readiness to learn. Identifies barriers to learning, and utilizes appropriate resources to facilitate learning. Assists patients to integrate the implications of illness and recovery into their lifestyles. Provides patient/family education to promote wellness, to reach their optimum level of function, and to prevent illness. Documents patient response to educational activities and modifies the plan as needed.

Continuum of Care:

Communicates and records pertinent information to appropriate individuals and/or departments/agencies that will provide the next level of care. Utilizes admission and discharge criteria to facilitate the transfer of patients to the appropriate level of care. Identifies needs and makes appropriate referrals to support services across the continuum. Collaborates with the health care team in implementing the plan of care from admission to discharge.

Improving Organization Performance:

Creates and promotes an environment conducive to teamwork and performance improvement. Participates actively in meetings and projects to improve the quality of service. Contributes to a culture of excellence by demonstrating self-motivation, adaptability, and reliability,

Leadership:

Participates in problem-solving and decision-making processes; and supports initiation of changes to enhance patient care. Demonstrates and maintains a forward-focused behavior. Creates patient assignments and delegates care that matches the competency of the staff to the needs of the patient. Communicates system problems and training needs to the Nurse Manager to ensure patient care outcomes are met. Participates in ongoing education and applies concepts to enhance practice. Coordinates multiple patients' needs and requests. Displays flexibility in a variety of work assignments. Precepts/orients new employees and students as assigned. Follows established procedures for reporting all incidents.

EDUCATION:

  • Must be a graduate from an accredited school of nursing.
  • BSN Degree preferred.

REQUIREMENTS:

  • Must possess a current and active NJ RN License.
  • Excellent communication skills.
  • Excellent observation skills.
  • Basic Life Support (BLS) from the American Heart Association.
