RN – On Call Hospice Visit Nurse – Full-Time Nights/Eve Rotating Shift Community Hospice – Saratoga

What you will do:

  • Based upon assessment of the On Call Triage RN, this On Call Visit RN will be called to care for Hospice patients/family primarily in their homes in the Saratoga and surrounding area.

Responsibilities:

  • Our RN on-call staff respond to symptom management needs of our Hospice patients and coordinate with our team to provide added support to patients and families. Initial calls are triaged by an experienced, trained Hospice Triage RN who manages calls and determines when the On Call Visit Nurse is required.
  • Teach patients/families regarding end of life & medications
  • Work with interdisciplinary team
  • Schedule is 40 hours per week
  • Friday 4pm – Saturday 8am (16 hours)
  • Saturday 4pm – Sunday 8am (16 hours)
  • Monday 12am – 8am (8 hours)

What you will need:

  • Valid NYS RN license
  • Associate’s degree in nursing. BSN preferred
  • Ability to learn/use an electronic medical record
  • Valid driver license and reliable vehicle
  • 2 years + experience as a RN in an acute care environment
  • 1+ year of experience as an RN in Hospice preferred
  • Strong understanding of Hospice principals preferred
  • Strong ability to assess priorities
  • Highest level of interpersonal skills to establish and maintain relationships with patients, families, physicians and coworkers
