Title: Case Manager/Family Specialist

Unit: Congregate Housing - Castle Gardens

Reports to: Senior Director for Congregate Housing

Location: Harlem, NY

Days/Hours: Monday – Friday (with a one designated late night per week)

Salary: $50,000--$55,000

Position Summary:

Responsible for engaging formerly homeless, just impacted tenant (s) and their family members residing in our mixed use supportive housing program, Castle Gardens through case management and supportive services.

Essential Duties and Responsibilities:

  • Conduct individual counseling sessions and home visits monthly;
  • Aid tenants in accessing and linking to community resources, including benefits, primary medical care, behavioral health, substance use treatment, legal services, education, vocational/educational services, and day care,
  • Coordinating care with ACS and/or Preventive Services as needed
  • Document direct service, collateral contacts on behalf of tenant in proprietary database accurately and in a timely fashion
  • Maintain case records to ensure audit readiness;
  • Work with tenants in developing individualized service goals;
  • Provide ongoing counseling, supportive services needed to achieve goals stated in individualized service plan;
  • Provide crisis intervention/counseling as needed;
  • Support tenants to ensure they complete annual recertification for housing subsidy;
  • Other tasks and responsibilities as assigned.

Qualifications:

  • Bachelor’s Degree in Human Services or related discipline;
  • Bilingual (English/Spanish or other language) preferred but not required;
  • Experience providing direct services to at risk families;
  • Experience using Microsoft Office programs and database case management systems;
  • Able to encourage disenfranchised populations to seek and obtain needed social services;
  • Strong follow-up, time management and organizational skills;
  • Ability to work well on your own and be self-directed, but also keep supervisor updated and work collaboratively with team members;
  • Strong verbal and written communication skills;
  • A valid driver’s license preferred;
