Job Description:

Community Care Worker I

Provides direct delivery and contract services to long term care services clients in assigned geographic area. Determines/re-determines need and eligibility for Community Care Services, authorizes and monitors Community Care Services provided by contract agencies. Work involves managing a caseload, interviewing elderly clients and clients with disabilities, documenting information, determining need for and amount of services, verifying case data, explaining program benefits and requirements, referring clients for appropriate services and assuring authorizations are registered on automated systems. Work includes operating a computer. Essential Job Functions:

Attend work on a regular and predictable schedule in accordance with agency leave policy. Interviews clients or resource persons and conducts home/site visits to assess the need for services. Obtains, verifies and calculates income and resources to determine client financial eligibility. Develops/coordinates/explains client services and payment plans and authorizes services to meet client's individual needs. Monitors services provided by contract agencies to measure the effect of services and to detect any problems. Conducts reviews to re-determine eligibility and functional needs. Prepares basic level ongoing and special narrative or statistical reports. Documents case records using automated equipment to form a record of each client. Prepares reports and records to provide management information on program operations. May review situations and provide temporary protective services to prevent or remedy abuse, neglect, or exploitation of elderly or disabled adults. Performs related work as assigned. Knowledge Skills Abilities:

Knowledge of customer services principles and practices. Ability to interview individuals and document essential information. Knowledge of Gerontology, the field of disability, the dynamics of the aging process and the special problems of the aged and disabled. Experience communicating effectively with all types of people, in all types of situations. Ability to operate computer equipment. Knowledge of business English. Education which demonstrates the possession of knowledge, skills, and abilities for job performance. Ability to listen to clients, exploring and reflecting feelings and present relevant alternatives describing feasibility and consequences of choices. Ability to complete work within required timeframes. Ability to appraise living conditions including physical and emotional environment. Ability to appraise nature and seriousness of threat to an elderly or disabled person. Ability to establish priorities, according to the relative importance and deadlines, set goals and manage time effectively. Ability to communicate orally and in writing. Ability to establish rapport with clients. Ability to communicate with community members to request assistance for clients, promote support for the agency or coordinate resources and programs. Registration or Licensure Requirements:

Valid driver’s license required. Initial Selection Criteria:

1) Application indicates any current customer service experience interacting with individuals or customers in a retail or social service setting, assisting customers and/or explaining benefits or products and/or screening individuals for services either by telephone or in person.

2) Application indicates applicant willing to travel a minimum of 65% of the time.

3) Application indicates applicants have attained at least a High School diploma or GED equivalent is required. Prefer attainment of at least Sixty (60) semester hours from an accredited college or university in the disciplines of Social Work, Psychology and/or Sociology.

4) Application indicates knowledge and/or experience in using general office equipment such as using a computer and software that may include Microsoft Office Products such as Outlook, Word, Excel, Power Point and SharePoint.

5) Application indicates applicant has the ability and/or experience to communicate effectively orally and in writing both in English. Bilingual in English and Spanish preferred.

6) Application indicates applicant can work a regular and predictable work schedule; specifically, the first shift, Monday -Friday, 8:00 am to 5:00 pm.

This position is included in a career track series and may be filled at any level within the career track, dependent on the selected applicant’s qualifications as they relate to career track promotion eligibility requirements.
