Description:

Come help lead our newest program at Trinity Health Of New England! This position is based out of Saint Francis Hospital but responsible for the entire region's practices.

Position Purpose

The Clinically Integrated Network (CIN) Community Health Coach acts as liaison between patients and healthcare and other community services, assisting patients to advocate for healthcare and social needs, and helping patients reach their highest level of wellness. Part of a larger population health team who are collectively accountable for coordinating patient care, improving health outcomes and disease management, closing quality gaps in care, and facilitating effective transitions through a continuum of care.

What you will do

  • Manage a patient caseload and provides education, screening, brief intervention, and/or facilitation of referrals to providers and community-based organizations and programs
  • Complete initial assessment for patients identified as rising risk or who have been referred for interventions to identify emotional and physical barriers to healthcare engagement
  • Partner with the patient, family, and care team to reduce the impact of the social determinants of health and improve patient's participation in wellness and early intervention prevention programs
  • Serve as local expert in navigating community services available
  • Collaborate with care team members, including the Primary Care provider (PCP) and their clinical support staff, as well as the overall CIN Population Health team to understand patient risks and quality gaps in care. This may include visits to aligned primary care offices to meet patients and collaborate with PCP on referrals and support services.

Minimum Qualifications

  • Current active Licensed Practice Nurse license.
  • Minimum three years experience in healthcare setting, working in fast-paced patient-facing environment.
  • Strong communication and customer service skills, especially in relation to engaging with patients and other healthcare professionals; experience using motivational interviewing and active listening techniques.
  • Proficient in basic computer software applications, specifically Microsoft Excel and documenting in Electronic Medical Records.
  • Community Health Worker Certificate highly desired, or experience with navigating or referring to local community resources a plus.
  • Bilingual Spanish/English desirable.
