The Social Worker is responsible for promoting the psychosocial and well-being of Collington residents by providing case management and other related support in collaboration with the Director of Social Services, in all levels of care, enabling continuity of care for residents and families and meeting regulatory requirements for licensed nursing beds in the health center. The Social Worker is also responsible for continuity of care to all residents for whom he/she is the primary designee.

Essential Functions

  • Assesses residents’ needs and provides case management and support to promote and maintain an optimal quality of life and wellbeing.
  • Within 30 days of admission obtain social history of new residents
  • Assists with implementing Advance Directive if not currently on file.
  • Coordinates individual service plans and assesses changes in residents’ psychosocial health needs.
  • Supports aging in place at all levels of the continuum, from Independent Living to Assisted Living.
  • Assesses residents’ psychological needs utilizing approved assessment tools, provides timely assessments, counseling, education, support services, referrals and meet with families as needed.
  • Completes appropriate documentation upon admission, transfer and discharge as required.
  • Communicates effectively with residents and families and assist with finding appropriate resources within the community.
  • Communicates with the Interdisciplinary Team regarding admissions, transfers, and discharges, scheduled family meetings, and other issues of common concern regarding the residents for whom the primary social worker and others as assigned by Social Services Director.
  • Assesses the needs of Independent Living residents utilizing psychosocial and other forms, such as the Montreal Cognitive Assessment (MOCA), Mini-Mental, Geriatric Depression Scale, Mini-Mental State Exam, etc., as needed.
  • Advocates for Residents Rights including abuse, neglect and exploitation and psychosocial/spiritual needs.
  • Coaches, supports, and enables other team members to respond to residents’ emotional and/or psychological needs.

Salary: Up to $90,000 based upon experience

Education

Required

Bachelors or better in Social Work.

Preferred

Masters or better in Social Work.

Experience

Required

5 years:

  • Bachelor’s Degree in Social Work, Master’s Degree preferred. • Minimum of five (5) years’ experience working in a geriatric setting or the equivalent combination of education and experience. • Knowledge of the geriatric population, family systems, regulatory requirements, and conflict resolution.
