Position Summary: The Staff Pharmacist is responsible for the safe, accurate, and effective use and dispensing of medications, as well as providing comprehensive pharmaceutical care to patients. Under the supervision of the Pharmacy Manager, the pharmacist reviews and verifies medication orders, provides clinical recommendations, participates in medication therapy management, and collaborates with providers, nurses, and other healthcare professionals to optimize patient outcomes. This role ensures compliance with federal, state, and organizational pharmacy regulations and policies, supports quality improvement initiatives, and provides guidance to Pharmacy Technicians and other staff as needed. The Staff Pharmacist also educates patients and healthcare staff on safe medication use and demonstrates strong clinical knowledge, attention to detail, and commitment to patient safety in both inpatient and outpatient settings.
Qualifications:
Primary Duties and Responsibilities:
1) Reviews, interprets, and verifies medication orders (verbal and written), transcribes to patient medication profiles, and dispenses medications accurately for inpatient and outpatient settings.
2) Monitors drug therapy regimens for contraindications, interactions, allergies, dosing appropriateness, and provide clinical recommendations to optimize patient outcomes.
3) Compounds and dispenses pharmaceuticals, including sterile products, IV admixtures, chemotherapy, and parenteral nutrition, following aseptic and safety protocols.
4) Manages pharmacy inventory, including ordering, stocking, monitoring expiration dates, and ensuring proper storage of medications and supplies.
5) Supervises, directs, and provides guidance to Pharmacy Technicians and support staff, including verifying daily activities and contributing to performance appraisals.
6) Monitors, manages, and optimizes complex medication therapies, including diabetes management, anticoagulation therapy, chemotherapy, and antimicrobial stewardship, by reviewing patient regimens, evaluating laboratory and clinical data, providing dosing recommendations, educating patients and healthcare providers, and ensuring adherence to evidence-based guidelines and safety protocols.
7) Collaborates with providers, nurses, and other healthcare professionals, providing drug information, clinical consultation, and clarifications as needed.
8) Educates patients, caregivers, and healthcare staff on safe and effective medication use, potential side effects, and therapeutic alternatives.
9) Documents all clinical activities, interventions, patient education, and pharmacy operations accurately and completely in the electronic health record (EHR).
10) Maintains compliance with federal, state, and organizational pharmacy regulations, including controlled substance management and documentation requirements.
11) Participates in Corporation meetings/committees, quality improvement initiatives, medication safety programs, medication use reviews, clinical audits, and data collection to enhance pharmacy operations and patient safety.
12) Stays current with emerging clinical guidelines, drug therapies, best practices, and participate in departmental training, education, and orientation programs.
Skills and Abilities:
1) Knowledge of pharmacology and therapeutics, medication preparation and administration, medication therapy management, medication safety and error prevention, and clinical guidelines and evidence-based practice.
2) Knowledge in a variety of federal and state laws and regulations that govern the practice of pharmacy, drug safety, patient privacy, and controlled substances.
3) Ability to accurately review, interpret, and verify medication orders, ensuring safe and effective patient care.
4) Ability to monitor drug therapy, identify contraindications, interactions, and dosing issues, and make clinical recommendations.
5) Ability to maintain accurate documentation in the electronic health record (EHR) and comply with regulatory requirements, including controlled substances.
6) Ability to prioritize tasks, adapt to changes in workload, and handle frequent interruptions efficiently.
7) Ability to work efficiently in a fast-paced environment while prioritizing tasks and maintaining patient safety.
8) Skilled in compounding and dispensing pharmaceuticals, including sterile products, IV admixtures, chemotherapy, and parenteral nutrition, following aseptic and safety protocols.
9) Skilled in collaborating with healthcare providers, patients, and caregivers, providing drug information, education, and therapeutic guidance.
10) Skilled in supervising, mentoring, and providing guidance to pharmacy technicians and support staff.

6) Occasional exposure to blood, body fluids or tissue, communicable diseases, chemicals, including but not limited to sticks or cuts by needles and other sharp items. Employee eligible for Hepatitis B Vaccination if desired.
