Discharge Coordinator - Full Time - Days

Responsible for coordinating care for patients following provider visits including booking follow-up appointments, coordinating and tracking referrals, and obtaining prior authorizations for services. Reviews and prepares patient charts for pending appointments, acquires records as needed, and performs various office functions. Serves a critical role in ensuring patients of Schuyler Hospital Clinics receive all of the necessary and recommended preventive, diagnostic and maintenance health services as prescribed by providers of the practice.

Roles and Responsibilities:

  • Performs care coordination process with patients following encounters, initiates follow up appointments, coordinates referrals, and arranges diagnostic testing including laboratory and radiology.
  • Responsible for insurance verification, verifies pre-authorization requirements based on insurance, obtaining prior authorization when appropriate and documentation of authorizations appropriately in the patients EMR.
  • Serves a critical role in "closing the loop" on referrals, imaging, and gaps in preventative care for patients by following up and documentation of pertinent clinical encounters thereby ensuring that the organization meets all quality incentive targets and the population receives the best possible care.
  • Receives documents and distributes patient and other communications to clinical and administrative team members in support of patient care. Messages are well documented, utilizing appropriate medical terminology and approved acronyms, clear, and accurate.
  • Files and scans completed documentation within the EMR, reconciles and attaches clinical reports within the EMR
  • Provides direct patient care and/or clinical support in the clinic to the provider when needed, rooming patients, gathers and documents vital signs,
  • Responsible for phone handling, receiving calls from patients, referring departments and offices and other customers, responsible for timely follow-up with all internal and external customers and patients, etc.
  • Attends departmental meetings, keeps up to date on office communication, and completes all assigned trainings
  • Other duties as assigned

Required Skills and Experience:

  • Medical Office Assistant Certification or Associate's Degree in an related field, may be replaced with work experience as outlined below
    1. Two years of relevant experience in a patient care/medical practice setting required; knowledge of medical terminology required. Experience as a care coordinator or case manager is helpful.
    2. Five years of work experience in customer facing role or service type of industry
  • Possesses strong oral and written communication skills,
  • Basic computer/office skills necessary,
  • Ability to multitask and navigate multiple computer platforms and/or electronic medical records
  • Strong customer service skills

Location and Travel Requirements:

  • Repetitive use of hands and fingers (ex: use of computer keyboard),
  • May require lifting and carrying light loads (up to 20 pounds), including boxes, supplies
  • Sitting, walking, or standing for long periods of time;
  • Must be able to physically operate the equipment used for the job

  • Onsite - Schuyler Hospital - 220 Steuben St, Montour Falls, NY 14865
  • Shift: (Mon-Fri) between the hours of 8:00 AM - 5:30 PM

Pay Range Disclosure

  • $20.50 to 26.00 per hour
