About the opportunity:
The primary purpose of the Case Manager – Young Women’s Support is to assist with the immediate and ongoing support needs of young women or those that identify as female (16-25), that are at risk of or experiencing homelessness predominately in the Southeast of Melbourne.
This primarily consists of providing a range of flexible support services and high-level coordination to clients, through a strength based and trauma informed, case management support model to maximize independence and minimize service dependence.
Young Women’s Support Service acknowledges and is designed to address the unique needs of young women at risk of entering or already linked into the youth homelessness system.
Using outreach and meeting young women where they are in the community, with a focus on housing and its stability, connection to community, support of multiple needs including but not limited to mental illness, substance abuse issues, engaging in education or employment and general wellbeing for support periods of up to 3 months.
The purpose of this role is to support young women and those who identify as female. As such, only candidates who are women or who identify as female will be considered.
Permanent full-time role (76 hours per fortnight) based at Cheltenham.
