Clinical Director- Family Medicine Physician

Position Summary:
The Clinical Director - Family Medicine Physician will assist the Medical Director in providing clinical leadership for Visiting Nurse Association of Central Jersey Community Health Center (CHC), ensuring highly reliable, evidence-informed, patient-centered care across all of our patient service sites.
Principal Responsibilities:
1. Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationships with colleagues up, down, and across the organization
2. Provides direct patient care for a minimum of 80% of working hours (4 work days/week) and administrative/clinical leadership support for the remaining 20%
3. Acts as an exemplar clinician by maintaining an active caseload of primary care patients in accordance with organizational policies, procedures, and protocols; models the way for the provider team in providing patient-centered care, fostering an environment of care grounded in teamwork, empathy, accessibility, and cultural humility
4. Functions as a Provider Electronic Health Record (EHR) Superuser, training new providers and collaborating with the clinical team to develop and deploy workflows
5. Participates in the development of relevant and realistic performance improvement plans and corrective action plans related to Quality Measure performance and QI/QA
6. Participates in the chart audit and peer review processes to ensure consistency of high quality care; provides feedback in the same
7. Develops agendas and leads effective and efficient meetings of the clinical and/or provider teams
8. Partners with the Medical Director and Chief Financial Officer and/or Controller to ensure productivity standards are defined, monitored, and met to ensure organizational viability, sustainability, and success
9. Participates in the triennial HRSA OSV and ensures continuous compliance with all applicable QRM standards as outlined in the HRSA Health Center Program and FTCA compliance manuals
10. Effectively communicates with all relevant stakeholders, prioritizing development of positive working relationships within the provider team
Competencies & Minimum Qualifications:
1. Graduate of an accredited medical school and residency program
2. Board certified in family or internal medicine
3. Licensure or eligibility to practice medicine in the state of New Jersey
4. Ability to obtain and maintain a federal and state controlled dangerous substance (CDS) registration
5. 3-5+ years of experience in the practice of medicine
6. 3-5+ years of ambulatory care experience, FQHC preferred
7. Understanding of population health and quality metrics/analytics
8. Excellent verbal, written, and interpersonal communication skills required
9. Bilingual English-Spanish preferred
10. Computer proficiency, data analysis skills, MS office required, including advanced Excel; or nursing informatics degree preferred
11. Car and valid NJ driver’s license

Sitting, fine motor hand movement, standing, walking, lifting 40 pounds, pushing, reaching, pulling; working with thermometer, sphygmomanometer, stethoscope, otoscope, ophthalmoscope, needles, syringes, EKG machine, venipuncture equipment, other diagnostic equipment, spirometry treatment, culture mediums, telephone, fax, photocopier, computer, printer.

In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $200,000.00 to USD $$296,000.00. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions. The total compensation range reflects the total package, including base salary, overtime, bonuses, add-on incentives, special clinical compensation, retirement plans, and all other benefits.
