Healthcare Supervisor (RN) – Day Habilitation Programs

Position Summary

The Healthcare Supervisor (RN) is responsible for the oversight and coordination of all health services within assigned Day Habilitation programs. This position ensures that nursing care is delivered in accordance with regulatory requirements, clinical best practices, and organizational standards. The RN promotes the health, safety, and well-being of all individuals served through ongoing assessment, direct nursing intervention, and collaboration with interdisciplinary team members.

Key responsibilities include completing and maintaining all required nursing documentation (quarterly, semi-annual, and annual nursing notes and assessments), developing individualized healthcare plans, providing staff training on health and safety topics, and ensuring timely follow-up on medical care. The Healthcare Supervisor serves as the clinical liaison between families, guardians, group homes, healthcare providers, and program staff to ensure effective communication and continuity of care.

Essential Duties and Responsibilities

  • Provide on-site nursing services including assessments, treatments, and medication administration in accordance with physician orders and agency policy.
  • Monitor the health status of individuals throughout the program day and respond promptly to changes in condition, illness, or medical emergencies.
  • Complete and maintain all required nursing documentation for individuals served, including quarterly, semi-annual, and annual nursing notes and assessments, as well as progress notes and health summaries.
  • Develop, implement, and maintain individualized healthcare plans, nursing care plans, and emergency response plans.
  • Coordinate and follow up on medical appointments, laboratory testing, and provider recommendations to ensure timely implementation of care.
  • Communicate effectively with families, guardians, physicians, and external providers to ensure continuity and quality of care.
  • Collaborate with the interdisciplinary team; including program directors, clinicians, and case managers in order to integrate health-related supports and goals into each individual’s Day Habilitation Service Plan (DHSP).
  • Provide education and in-service training to program staff on relevant health topics such as symptom recognition, infection control, universal precautions, and emergency procedures.
  • Maintain accurate and timely nursing documentation in compliance with agency, DPH, and MassHealth requirements.
  • Participate in case reviews, incident investigations, and interdisciplinary team meetings.
  • Monitor and maintain medical supplies, first aid kits, and ensure the safety and functionality of medical equipment.
  • Participate in on-call nursing rotation and provide coverage during nursing call-outs as needed.

Qualifications

  • Current Massachusetts Registered Nurse (RN) license in good standing.
  • Minimum of two (2) years of nursing experience; experience with individuals with developmental or intellectual disabilities preferred.
  • Demonstrated clinical assessment, critical thinking, and documentation skills.
  • Ability to work independently, prioritize tasks effectively, and collaborate within an interdisciplinary framework.
  • Comprehensive knowledge of DDS, DPH, and MassHealth regulations governing Day Habilitation programs.
  • Excellent written and verbal communication skills.
  • Valid driver’s license and reliable transportation required.

Physical and Environmental Requirements

  • Frequent movement throughout the program to observe and assist individuals.
  • Ability to lift, bend, and assist with physical care as required.
  • Exposure to individuals with diverse medical and behavioral needs in a dynamic day program environment.
  • Must be able to respond to medical emergencies quickly and effectively.
