Summary:
The Care Transition Coordinator / Patient Transportation Specialist facilitates smooth transitions for clients from healthcare facilities to their homes. This role involves managing discharge processes, ensuring safety during transportation, and providing follow-up support. You will collaborate with interdisciplinary teams, maintain accurate documentation, and utilize company-issued tools to ensure compliance and optimize client care. This role requires strong communication skills, confidentiality, and a commitment to safety and client well-being.
Pay: $16 - $17/hr
Shift: Sunday - Thursday 7:00 am - 3:00 pm
Note: The primary responsibility of this position is driving, with 90% of the time spent on driving.

Excellent oral and written communication and interpersonal skills. Sound clinical judgment and excellent clinical skills. Ability to problem-solve by gathering and analyzing information. Ability to handle a crisis or potential risk situation and react appropriately. Working knowledge of federal and state standards as well as regulating body and compliance standards. Comfortable working independently and as part of a team. Strong interpersonal skills and the ability to communicate effectively with clients, families, and colleagues. Ability to handle multiple tasks, remain organized, and manage a dynamic schedule.
Physical Demands:
While performing the duties of this job, the employee is frequently required to talk or hear. The employee is regularly required to sit, use hands to finger, handle, or feel, and reach with hands and arms. The employee is occasionally required to climb stairs, stand, walk, stoop, twist, kneel, or crouch. The employee must occasionally lift and/or move up to 25 pounds and seldom lift and/or move up to 50 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus. Work Environment: The noise level in the work environment is usually moderately quiet and indoors. This position will require local travel and will be scheduled based on operational needs. Incumbent may be exposed to virus, disease, and/or infection from clients in the work environment. Incumbent may be exposed to traumatic situations (e.g., psychiatric).
