How You’ll Make a Difference (Essential Functions)

The Palliative Care Coordinator is responsible for a variety of clinical and administrative tasks for the Agrace Palliative Care program. This includes but is not limited to conducting phone conversations with potential clients to answer questions about services, scheduling palliative care clinician visits, coordinating care with referral partners, and other administrative support in the Supportive Care program.

This role requires exceptional communication skills as he/she/they will interact with patients/families, account managers, primary care providers and other healthcare partners along with the Palliative Care team.

Key Responsibilities

  • Coordinating Care for the Palliative Care Patient
    • Conduct phone conversations with patients and families to answer questions about Agrace values, services, benefits, and philosophies and provide education on the benefits of Supportive Care
    • Process incoming referrals and/or clinical information and document this in the electronic medical record
    • Coordinate palliative care consults with patients and/or patient representatives
    • Receive incoming communication from both patients and clinical staff in the field, relay messages and provide troubleshooting.
  • Review and maintain patient medical record
    • Maintain current patient information within the patient’s chart, including demographic information, medical information, and other relevant data metrics
    • Identify and request records missing from the patient’s chart
  • Other duties
    • Provide miscellaneous support, including cross-coverage training for other Supportive Care operational tasks
    • Participate in internal work groups, meetings, and committees relative to position responsibilities.
    • Complete mandatory educational requirements

You Bring (Qualifications)

  • High school diploma/equivalent; post high school education is preferred
  • Completion of an accredited Medical Assistant program or CMA, preferred
  • Minimum one year of clinic experience is preferred
  • Previous scheduling and administrative experience preferred
  • Experience with EMR (electronic medical records), medical terminology
  • Ability to develop, maintain, and apply knowledge of palliative care.
  • Knowledge of medical terminology.
  • Excellent verbal and written communication skills.
  • Ability to maintain professional boundaries with patients, families, and co-workers and sensitively meet the needs of terminally ill patients and their families.
  • Ability to work effectively and cooperatively with others
  • Ability to prioritize and thrive in a fast-paced environment
  • Ability to think creatively and demonstrate effective problem-solving skills.
  • Competence in culture/ethnicity, gender, race, age, identification, and educational background diversity
  • Comfortable with Microsoft Office Suite and experienced with electronic health records (EHR); Epic preferred
  • Ability to be flexible in time and task to meet organizational needs
  • Ability to maintain confidentiality about patients, families, caregivers, staff, donors, and organizational intellectual property.
  • Ability to use proper body mechanics and standard precaution
