Licensed Practical Nurse II - (2500071K)
The BMSC Adult Community Clinical Services (ACCS) Program is seeking a dynamic candidate who will serve as a Licensed Practical Nurse on one our integrated teams. The LPN II will provide supports & services to adults with major mental illness in Department of Mental Health ACCS Group Living Environment residential homes as well as in the community. The LPN II will provide face-to-face community-based services to assigned persons in their homes, homeless shelters, in hospital settings or any other setting in which the person served resides or currently requires intervention (including in one of the BMSC ACCS 3 24/7 group residential environments). The LPN II will provide behavioral and physical health monitoring and support as well as medication administration and education as needed. The LPN II will be responsible for completing Physical Health Assessments, participating in the development of treatment plans, and providing trauma-informed, person-centered community-based interventions that target skill development in the areas of Health & Wellness. The LPN II will conduct routine assessments and provides medical care as needed, while also ensuring that appropriate medical services are accessed by collaborating with care coordination entities.
Please Note: This position is 40 hours per week, Monday to Friday from 8:30 am to 5:00 pm. The scheduled days off will be on Sunday and Saturday.
Employees in this position will be required to perform mandatory overtime.

MINIMUM ENTRANCE REQUIREMENTS:
Applicants must have at least one year of paraprofessional experience in the field of nursing as a licensed practical nurse.
SPECIAL REQUIREMENTS: Possession of a current and valid license as a practical nurse under the Massachusetts Board of Registration in Nursing.
Salary placement is determined by a combination of factors, including the candidate’s years of directly related experience and education, and alignment with our internal compensation structure as set forth by the Human Resources Division’s Hiring Guidelines. For all bargaining unit positions (non-management), compensation is subject to the salary provisions outlined in the applicable collective bargaining agreement and will apply to placement within the appropriate salary range.
