Responsibilities

The Family Connects Program Support Specialist works with a team of nurses to assist in coordinating Family Connects cases, including scheduling home visits for families in the birth hospital, making phone calls to schedule families missed during the hospital visits, scheduling and coordinating visits and assignments for the nurses, fielding our public information lines to answer questions and reschedule parents, and conduct post visit follow-up phone calls with each family for the purpose of monitoring customer satisfaction and tracking referral outcomes. PSSs will work in English and/or Spanish.

This position requires providing service to the following age population(s) 0-50_________ in a manner that demonstrates an understanding of the functional/developmental age of the individual served.

Qualifications

  • Graduation from four-year college with a degree in the human services area preferred.
  • Excellent verbal and written communication skills with strong attention to detail and multitasking skills.
  • Demonstrated ability for collaborative work in a team environment, as well as ability to work independently to meet expected deadlines.
  • Provision of reference from most recent work supervisor plus two other prior workplace supervisors or colleagues.
  • Ability to accept personal differences, to establish trusting relationships, and to work with culturally diverse populations.
  • Knowledge of different community resources and agencies. Spanish fluency highly desired.

Conditions of Employment

Since this position will require the selected candidate to engage in regular contact with children in the form of care, guidance, supervision or training, you will be required to meet the requirements of the Child Protective Services Act, including the Pennsylvania State Police criminal background check, the Pennsylvania Department of Welfare child abuse criminal background clearance, and an FBI finger print based federal criminal records check. The selected candidate will not be allowed to commence working until all three (3) are completed satisfactorily.
