Job Description

PURPOSE OF POSITION:

Support the Facility's finances by performing the functions consistent with timely Clinical Data abstracting and Coding ( CPT & ICD 10-Cm and PCS) thereby positively impacting/ the AR days.

AREAS OF RESPONSIBILITIES:

Review, abstract and accurately code and monitor selection of Principal and Secondary Diagnoses and procedures according to Coding Guidelines which ultimately impacts the Facility's finances.

SUMMARY OF DUTIES AND RESPONSIBILITIES:

  • Reviews the medical documentation identifies all diagnoses and procedures and assigns ICD-10-CM Diagnoses and / or Procedure codes or CPT Codes as appropriate to the patient class.
  • Assign primary and secondary diagnoses and sequence according to the official coding guidelines and unified hospital discharge data set.
  • Assign evaluation and management levels as appropriate for outpatient ED and Amb/Surg correct all 300 edits using the EPIC system in keeping with correct coding/ initiate edits. Monitor discharges not final billed accounts. (DNB)
  • Communicate the lack of documentation to physicians. Identify congenital malformations on the ED records appropriately. Identify sterilization, abstracting and coding for submission to DOH.
  • Perform daily monitoring of WQ for priority coding and excess time holding for documentation. Perform quality monitoring on the coding as required.

Minimum Qualifications

1. High school diploma or its educational equivalent, approved by a State’s Department of Education or a recognized accrediting organization; and

2. Completion of a coding or billing, medical record technology or health information technology (HIT) program approved by a nationally accredited agency (e.g., the American Health Information Management Association (AHIMA) or the Commission on Accreditation for Health Informatics and Information Education (CAHIIM)); or

3. Certified Coding Specialist (CCS) by AHIMA, Certified Professional Coder (CPC) by the American Academy of Professional Coders (AAPC) or Certified Medical Billing Specialist (CMBS) by the Medical Association of Billers (MAB); or

4. Two (2) years of satisfactory experience in coding and/or abstracting medical records in a health care setting.
