About the Team:

Sound Physicians formed a Medicare Accountable Care Organization called Sound Physicians Long Term Care Management, LLC (SLTCM) which is a Medicare Shared Savings Program (MSSP) Accountable Care Organization (ACO) for Medicare Beneficiaries who reside in long-term care (LTC) facilities.

Sound’s ACO model is focused on leveraging Sound’s national footprint, innovation, proven experience with value-based care to partner with SNFists and long-term care facilities to achieve coordinated care, cost efficiencies and quality outcomes for long-term care residents.

In the ACO model, once Medicare quality and performance targets are met, participating physicians will share in the Medicare program savings from CMS through the ACO.

About the Role:

The Clinical Data Coordinator will work closely with the Director of Data Management and the Associate Chief Medical Officer (ACMO) to close quality care gaps in quality care opportunities and will contribute to the optimal performance and efficiency of the program. The Clinical Data Coordinator is a part of SLTCM ACO and will collaborate with clinicians, health systems, internal departments, and third-party vendors.

The Clinical Data Coordinator will be responsible for collecting documentation from electronic medical records to support quality care gap closure through medical record reviews. The Clinical Data Coordinator is responsible for contributing to improved workflow processes, communications, and standards, as well as initiatives for operational, financial, and clinical performance. This position will report to either the Director of Data Management or Associate Chief Medical Offer (ACMO).

The Details: This is a full-time, remote, work-from-home opportunity. No travel required.

In this role, you will be responsible for:

  • Developing effective relationships with provider group and facility data owner and collaborating across departments to ensure timely collection of data
  • Ensuring all quality data is collected and coded accurately
  • Communicating effectively with stakeholders, clinical facilities, and third-party vendors
  • Performing data collection, data entry, and quality performance monitoring activities
  • Closing quality care gaps by mining EHR performance data and collecting performance data from disparate sources across platforms
  • Providing information, resources, and scanning relevant components of the medical record to support reviews performed
  • Understanding the quality care gap closure process
  • Maintaining confidentiality and adhering to HIPPA requirements

Experience:

  • 1+ years of experience in a healthcare background with medical terminology and familiarity of clinical issues
  • Proficient experience with Microsoft Word and Excel
  • Experience with MIPS, ACOs, and other Medicare Alternative Payment Models

Pay Range: $20.00-$30.00 hourly. Exact pay will be determined based on candidate experience and geographical location.
