About the Role

You will provide support for patients living with type 1 and type 2 diabetes, including education and advice about management of the condition in conjunction with Medical Officers and Endocrinologists

  • You will provide comprehensive diabetes self-management education to support participants to meet goals of the research projects
  • You will use a systematic approach to assess an individual’s mental, physical, functional and cognitive status in order to inform their clinical care planning
  • You will monitor progress towards desired goals and the outcomes of all interventions
  • You will refer (or recommending referral where appropriate) to other specialist nursing, medical or allied health professionals as required
  • You will provide direct clinical care and interventions
  • You may assume or be delegated case management roles in designated programs or for specific aspects of diabetes care, such as insulin initiation and stabilisation, insulin pump therapy, Continuous Glucose monitoring

Classifications for this position will be ZJ4 ($113,646 Per Annum, Pro Rata)

Your Contribution

Qualifications:

  • Bachelor of Nursing or equivalent
  • Registered as a Division 1 with the Australian Health Practitioner Regulation Agency (AHPRA) and current Practicing Certificate
  • Graduate Certificate in Diabetes Education

Skills/experience:

  • Minimum 5 years post graduate nursing experience
  • Demonstrated advanced clinical nursing skills and knowledge Demonstrated autonomous practice
  • Demonstrated ability to work in a multi-disciplinary team
  • Knowledge of workplace statutory and regulatory requirements and their impact upon the speciality of diabetes education
  • Must hold a qualification as a Credentialed Diabetes Educator with a minimum of 5 years’ experience
  • Knowledge of and commitment to quality improvement
  • Current National criminal history check, or willing to obtain

Diabetes Research & Technology Group

Located at St Vincent's Hospital in the Department of Medicine (DoM), Faculty of Medicine, Dentistry and Health Sciences, the appointee will be responsible for the effective protocol implementation of diabetes clinical research at St Vincent’s Hospital and collaborating centres. This research will primarily focus on technology and its interface with people living with diabetes.
