Position Summary

Applies the nursing process with patients and families, integrating principles that reflect sensitivity to their age-specific/developmental needs, gender, culture, religion, psychosocial and socioeconomic backgrounds in order to provide evidence-based, holistic care in a manner that respects the worth and dignity of each individual. Practices professional nursing as a licensed practical nurse within the legal and ethical framework established by the Connecticut Nurse Practice Act and the American Nurses Association Scope and Standards for Nursing Practice and Code for Nurses, American Nurses Association Home Health Nursing, National Hospice Association and applicable regulatory agencies.

Essential Duties & Responsibilities

  • Triages urgent calls from patients that are too complex for medical assistants including but not limited to medical issues, reactions to medications, IVs, and other treatments. Communicate with the provider as needed.
  • Administer injections and/or prescribed filled medications and assist with other basic medical care tasks under the direction of the provider
  • Assists providers with office based procedures
  • Collect samples for lab testing as needed
  • Obtain patient medical history including allergies and and relevant clinical information, document as needed in EMR
  • Assure all documentation, results, reports pertinent to the visit are available to the provider for review with the patient
  • Document all patient information obtained in the Electronic Health Record according to policies and procedures for EHR use.
  • Processes medication refills
  • Provide emotional and psychological support to patients as needed
  • Communicate with patients to provide advice, comfort and education on health related matters consistent with scope of practice and provider direction
  • Complete work tasks in EMR as needed
  • Other job related duties as assigned.
  • Upholds both the Connecticut Nurse Practice Act and American Nurses Association Scope and Standards of professional practice.

Minimum Qualifications

  • Graduate of an accredited Licenced Practical Nursing program
  • Currently licensed as a Licensed Practical Nurse in the State of Connecticut.
  • Has the knowledge and skills necessary to assess health needs of patients and to perform nursing care in a safe, caring manner.
  • Willingness to obtain BLS certification within six months of hire

Preferred Qualifications

  • One year of experience in an office based physician practice
  • One year of experience working with electronic health records
  • Active BLS certification
