Description
Provide leadership and supervision of employees, providing work direction and guidance to the In-Home Services 245D Service Coordinators. Lead the 245D In-Home Service Coordinator team in accordance with provider and program requirements as defined by the Minnesota Department of Human Services, Minnesota Legislator and Minnesota State and County Contracts. Use process improvement techniques and methodologies to improve Lifeworks’ In-Home and 245D licensed services that result in program compliance, improved customer satisfaction, participant growth and enhanced financial performance.

Hours: Full-time 40 hours/week, Monday – Friday (Exempt)
Location: Lifeworks Services – Richfield / 20% in-person (Hybrid)
Compensation: $56k - $59k yearly

How you’ll be spending your time:
- Taking a proactive role in leading, participating in change management, fostering innovation, and promoting cross-training to continuously enhance Lifeworks’ service delivery, ensuring consistently high levels of customer satisfaction.
- Developing and instilling processes and procedures to maintain high customer service standards for the In-Home Services Teams. Leading process improvement initiatives and projects to improve the delivery of In-Home Services.
- Providing leadership and guidance to the In-Home service team personnel establishing key performance metrics including program compliance, customer satisfaction, problem resolution times, call productivity, administrative tasks, etc. Supporting the recruitment, selection, development and engagement of staff members.
- Monitoring and reporting metrics of quality over program compliance, customer satisfaction and achieving goals in Lifeworks strategic business plan.
- Defining compliance requirements for assigned service lines against all administrative rules and regulations, including compliance with government contracts, laws and regulations. In alignment with the Program Leadership team, maintain Lifework’s quality standards; procedural expectations; day-to-day work assignments and staffing adjustments.
- Being a subject matter expert for all of Lifeworks’ service offerings. Maintaining a deep understanding of general services structure and how to access services and provide mentorship to individuals across the organization seeking to better understand the Self-Directed and In-Home service lines and the markets we compete in. Communicating service line priorities to the Service Coordinator team daily and support communications efforts of the In-Home Leadership team.
- Carrying a modified 245D Caseload of 30-40% which includes coordinating services for assigned caseload including scheduling, organizing, participating in, and documenting all requirements per MN 245D and 256B license or Lifeworks practices by collaborating with the Expanded Support Team and others as appropriate. It is anticipated that this supervisor role could be required to perform the duties of the Service Coordinators as needed to meet the needs of the service lines.
- Following safety procedures and assisting in identifying safety needs for self and others. Performing all other duties as assigned.
Requirements
- Bachelor’s degree in human service field or related field with one year experience, two-year degree in a human service field and three years’ experience, or a combination of education and experience to equal four years.
- Customer service experience strongly preferred.
- Fluent in English, verbal and written.
- Proficiency in Microsoft Office Suite.
