Master Case Manager - Wilmington

Schedule: Monday to Friday 9am to 5pm
Pay Rate: $29.76 per hour
Job Summary:
As the Master Case Manager, you will be responsible for providing comprehensive case management services to individuals and families in need. You will assess client needs, develop personalized care plans, coordinate services, and advocate on behalf of clients to ensure they receive the resources and support they require. Your expertise in navigating complex systems and your ability to problem-solve will be crucial in this role.
Key Responsibilities:
- Conduct comprehensive assessments to identify client needs and develop individualized care plans
- Coordinate and monitor the delivery of services, including healthcare, social services, and community resources
- Advocate for clients to ensure they receive the appropriate level of care and support
- Maintain detailed case notes and documentation to track client progress and outcomes
- Collaborate with a multidisciplinary team of professionals to provide integrated care
- Participate in ongoing training and professional development to stay current with best practices in case management
Qualifications:
- Bachelor's degree in social work, human services, or a related field
- Minimum of 5 years of experience in case management or a similar role
- Demonstrated expertise in navigating complex social service systems
- Strong communication and interpersonal skills, with the ability to build rapport with diverse clients
- Proficient in data management and record-keeping
- Certification in case management or a related field is preferred
- Familiarity with government social welfare programs and regulations
