General Purpose of Job:

Provides leadership and direction for the Case Management area in the department of Patient Support Services. By translating the Hospital and Health System's vision and strategies into practice, the Manager contributes to high quality, compassionate, cost-effective services provided to patients.

The Manager works in collaboration with Administrative staff, Directors, Managers, Physicians and other Sparrow Caregivers to achieve care coordination and integration of services approach for the patient’s experience of care.

Essential Duties:

  • Participates in efforts to support and advance the Health System's mission, vision, strategies and goals.
  • Provides leadership to the Case Management area of Patient Support Services Department (PSS).
  • Works in conjunction with executive and clinical leadership, administration, other staff, and external agencies, and, as appropriate, will advise regarding current and projected trends, areas of deficiency and significant activities related to the functions of case management.
  • Facilitates and fosters the development of a collaborative relationship between case management, nursing, other departments and the medical staff.
  • Represents the Hospital and Health System to the community, educational institutions, professional organizations, payers and others.
  • Ensures full compliance with regulatory and accrediting agencies.
  • Develops and implements short and long term goals for case management that reflect the Health Systems strategic vision and goals.
  • Selects, orients, mentors and evaluates department staff members.
  • Maintains a high level of staff morale, professionalism, efficient and effective performance, motivation and pride in the services performed for patients and the Health System.
  • In conjunction with the Department Director, appropriately plans for and projects the annual case management financial plan. Monitors the results and advises leadership when there is a need to adjust the plan.
  • Monitors customer satisfaction, performance improvement and corporate compliance indicators, and key indicators of effectiveness and efficiency to ensure optimal performance.
  • Participates in establishing policies and procedures, position descriptions, staffing needs, staff scheduling, and productivity.
  • Assists in the development and implementation of special projects.
  • Supervise processes and personnel directly involved in securing authorization and financial funding from payers for discharge planning needs.

Job Requirements

General Requirements

  • Current State of Michigan Registered Nurse license OR Current State of Michigan Licensed Master Social Worker • Certification in Case Management, CCM or ACMA - Preferred

Work Experience

  • Minimum of 3 years acute care hospital experience providing care for patients

Education

  • • Bachelors Degree in Nursing OR Licensed Master Social Worker

Specialized Knowledge and Skills

  • Possess strong communication, negotiation, and influencer skills. • Computer skills including electronic health records and Microsoft office (Word, Excel, PPT). • Demonstrates the ability to work collaboratively across the health care continuum with a variety of people. • Comfortable with presenting information to a wide variety of constituents.
