Position Summary

Under the direction of the Medical Director (or designee) provides nursing care to patients with bleeding and clotting disorders, coordinates activities of the clinic and the professional treatment team, assists in coordination of research, and provides education regarding bleeding and clotting disorders and home therapy to patients, their families, health care practitioners and the public. This position may be assigned primarily to either clinics or home care and is expected to perform most responsibilities independent of direct supervision

Responsibilities

  • MAJOR RESPONSIBILITIES * Provides and documents nursing care to patients with bleeding and clotting disorders, assessing patients, implementing the treatment plan developed by the professional treatment team, evaluating the patient’s response and modifying plan as needed. * Acts as a consultant to hospital inpatient and outpatient medical providers while patients are being cared for at community hospitals. * Provides first level phone triaging of patients according to the severity and complexity of their symptoms. * Establishes and maintains patients on the home therapy program through education of the patients and families to obtain initial and continuing certification in home therapy; following up on any special needs; supervising home care product and prescription monitoring/ordering; and intermittently assessing the advisability of maintaining patients on the home therapy program and/or maintains patients on anticoagulation therapy, monitoring lab values and communicating with the patient and medical team as needed. * Provides timely management of surgical and other medical procedures for bleeding and clotting disorders, including coordination with Therapeutic Products and the Home Care Program, with preop clinic and all other providers. * Collaborates with the Department of Benign Hematology at Froedtert Hospital to coordinate care for adult patients with bleeding and clotting disorders. * Functions as a primary educator of patients and families with bleeding and clotting disorders by providing information regarding pathophysiology and treatment, and responding to patients’ and families’ questions and concerns. * Acts as a liaison between the patient, his family, the physicians and other members of the treatment center, as well as acting as a resource to the patient’s primary care physician and other health care providers involved in the patient’s care. * Provides education to health care practitioners and the general public regarding bleeding and clotting disorders through presentations/in-services to community organizations, schools, and health care providers, and by developing educational materials including newsletter and website articles. * Coordinates the hemophilia and /or coagulation clinic and services provided by the treatment center team members including: the scheduling/organizing of the Comprehensive and Diagnostic/Coagulation Clinics, scheduling of patient appointments, and reviewing of outgoing correspondence and treatment recommendations. * Assists as needed in the coordination of research and prevention activities involving the patient population, including coordinating laboratory studies and data compilation/maintenance, as well as educating patients and families regarding the purpose of the research. Coordinates the collection of data for state and federal grant purposes by collecting the Comprehensive Center for Bleeding Disorders’ data on a quarterly or as- needed basis. * Maintains current knowledge base and skills through participation in continuing education, professional meetings and conferences, and reviewing current literature.
  • GENERAL DUTIES * Functions as part of a comprehensive team, provides cross coverage for other team members, and provides assistance with process development, implementation of new programs and other initiatives, and other projects/duties as assigned. * Assists in maintaining patient files / medical records filing system * Provides assistance to Clinical Secretary and other CCBD staff as required. * Provides assistance to Manager, CCBD and other BCW employees to support the distribution and reimbursement of factor replacement products.

Qualifications

Education

  • Bachelor's Degree in Nursing preferred
  • Associate's Degree in Nursing required
  • Nursing Diploma plus equivalent relevant experience required

Experience

  • 1-3 years nursing experience required
  • 1-3 years home care and/or pediatric nursing experience preferred for CCBD nurse responsible for home care preferred

Knowledge, Skills and Abilities

  • Excellent organizational ability with attention to detail required
  • Ability to work independently under minimum supervision as well as to function as a productive team member required
  • Excellent triage skills required required
  • Excellent verbal and written communication, public speaking, and teaching skills required
  • Strong interpersonal skills required
  • Working knowledge of computer databases and word processing applications required
  • Valid Wisconsin driver’s license and access to a vehicle required
  • Venipuncture skills required
  • Current CPR certification required

Licenses and Certifications

  • Current Wisconsin RN licensure required

  • Basic medical equipment and supplies needed for direct patient care and teaching required
