San Mateo County Health - San Mateo Medical Center is seeking a qualified Social Worker III. The Social Worker works alongside the clinic healthcare team, the Health System, and collaborative County Departments. The Social Worker will assist SMMC patients assigned to the Ron Robinson Senior Clinic (RRSCC), serving older adults address the social/medical barriers that impact their health. RRSCC’s patient population consists of individuals, predominantly 60+ year old with: moderate to severe medical conditions; have mental health diagnosis'; are high utilizers of services; those who need support at home; may require out-of-home placement, and who may need end of life care. The Social Worker will provide intensive case management, linkages to supportive services, and will collaborate with County and community partners to ensure a patient receives patient-centered care and timely services.

The Social Worker will make as needed home visit to determine medical needs and in providing case management duties. The Social Worker will travel throughout the County as needed, to conduct home visits, attend meetings, and perform other duties of the position. The Social Worker will conduct intake and on-going case management activities, coupled with warm-hand offs in the clinic. Some of those activities include:

  • Complete necessary documents, paperwork, and work within different information technology systems, like EPIC Systems, Avatar, and Q software.
  • Conduct as needed monthly follow up with patients.
  • Provide education, outreach, and community resources information to patients, family members and caregivers.
  • Participate in quality improvement efforts.
  • Engage in interdisciplinary team meetings or other activities to support San Mateo County's pilots.

The ideal candidate will possess the following:

  • Master’s degree in Social Work or Clinical Psychology.
  • Two years+ of experience working with aging adults, palliative care, hospice, or related scope of work preferred.
  • Fluency in Spanish preferred.
  • Ability to independently develop and implement sound case plans.
  • Experience balancing client-centered case coordination and the accompanying administrative documentation.
  • Superior organizational skills to manage an active high-volume caseload and related documents.
  • Proficient computer skills to use several independent data base systems.
  • Excellent oral communication and writing/documentation skills.
  • Ability to coordinate and facilitate case conference meetings with various community partners.

Examples Of Duties

Duties may include, but are not limited to, the following:

  • Provide intensive, highly skilled casework services with patients or clients and their relatives.
  • Interview clients or patients and evaluate problems presented.
  • Prepare and implement comprehensive treatment or care plans.
  • Consult and cooperate with other divisions of the department and with community agencies and groups in carrying out treatment plans.
  • Prepare social histories with emphasis upon psycho-social factors affecting patients or clients.
  • Assist in development of community resources when necessary to carry out treatment plans.
  • Provide therapeutic and/or protective services to patients, clients, and community groups in individual or group situations.
  • Interpret programs and consult with individuals and various community groups to provide a better understanding of social aspects of environmental, emotional and physical problems and appropriate methods of alleviating them.
  • Assist in the development of training for other staff.
  • Interpret the rules, regulations and policies of the department to clients and applicants.
  • May serve as lead supervisor.
  • Prepare and maintain casework records, reports, and relevant correspondence.
  • Perform related duties as assigned.

Qualifications

Licensure/Certification:

  • California class C driver license or equivalent.

Education:

  • Satisfactory completion of 36 semester units of social welfare and/or appropriate behavioral science courses

Experience:

  • Three years’ experience performing duties comparable to those of a Social Worker II or higher in San Mateo County.
  • Substitution: Possession of a Master's Degree in Social Work from an accredited school of Social Work and two years’ experience performing duties comparable to those of a Social Worker II or higher in San Mateo County.

Knowledge of:

  • Theories of human behavior and their practical applications.
  • Psycho-social, medical, and economic interrelationships that result in individual and social abnormalities.
  • Philosophy and function of social work systems.
  • Appropriate utilization of a variety of social resources.
  • Laws, rules and regulations governing the operation of social work agencies, specifically those relating to the specific areas of assignment.

Skill/Ability to:

  • Formulating, developing and implementing treatment plans.
  • Working effectively with persons differing in value systems, ethnicity, cultural backgrounds and psychopathologies.
  • Working with a variety of social resources to plan and coordinate services to individual clients.
  • Acting as a consultant and trainer to other social service staff.
  • Preparing clear and concise reports.

Current County of San Mateo and County of San Mateo Superior Court of California employees with at least six months (1040 hours) of continuous service in a classified regular, probationary, extra-help/limited term positions prior to the final filing date will receive five points added to their final passing score on this examination.

The examination will consist of an application screening (weight: pass/fail) based on the candidates' application and responses to the supplemental questions.an interview (weight: 100%). Depending on the number of applicants, an application appraisal of education and experience may be used in place of other examinations or further evaluation of work experience may be conducted to group applicants by level of qualification. All applicants who meet the minimum qualifications are not guaranteed advancement through any subsequent phase of the examination.All examinations will be given in San Mateo County, California and applicants must participate at their own expense.
