Location: Troop 6: Kirkwood Highway, Wilmington De

Schedule: Various Hours 40 hours per week.

Pay Rate: $30 to $33 per hour depending on experience

Job Summary:

As a Behavioral Health Case Manager III, you will play a crucial role in coordinating and overseeing the delivery of comprehensive behavioral health services to our clients. In this senior-level position, you will leverage your extensive expertise to assess client needs, develop personalized care plans, and collaborate with a multidisciplinary team to ensure seamless service provision.

Key Responsibilities:

  • Conduct comprehensive assessments to evaluate clients' behavioral health needs and develop individualized treatment plans
  • Coordinate and monitor the implementation of care plans, ensuring clients receive the appropriate services and support
  • Collaborate with healthcare providers, social workers, and community organizations to facilitate access to resources and services
  • Advocate for clients' rights and needs, serving as a liaison between clients and service providers
  • Maintain detailed case records and documentation in accordance with agency policies and regulatory requirements
  • Provide supervision and mentorship to junior case management staff, sharing your knowledge and best practices
  • Participate in ongoing training and professional development to stay current with industry trends and best practices

Qualifications:

  • Bachelor's degree in social work, psychology, or a related human services field
  • Minimum of 5 years of experience in behavioral health case management, with a demonstrated track record of success
  • Licensure or certification as a Certified Case Manager (CCM) or Certified Rehabilitation Counselor (CRC) preferred
  • Excellent communication, interpersonal, and problem-solving skills
  • Proficient in using case management software and electronic health record systems
  • Ability to work effectively in a team-based environment and collaborate with diverse stakeholders
