Program Description
The Coordinated Care Agency is fully empowered to design and deliver innovative, welcoming, and comprehensive services and housing for people experiencing deep poverty and homelessness. The purpose of our work is to ensure people have access to healthcare and supports they need to thrive. The team works to promote social changes to dismantle systemic racism, injustices and inequities that contribute to housing instability.
Within our permanent supportive housing programs in King County (Seattle, Kent and Renton) we offer over 775 single resident occupancy units to adult men and women. We provide 24-hour staff support to our residents using a Housing First and Harm Reduction approach.
Position Description
We have an opening for a Case Manager position at our Frederic Ozanam house program. Our program serves unhoused men, 55yrs and older, with a disabling condition. These positions are full-time (40 hours weekly).
We believe that every person has the right to a safe, affordable place to call home. We seek a case manager who care deeply about the health and well-being of our residents who have lived through the trauma of homelessness.
The case managers in our permanent supportive housing programs works independently and as part of a team to provide outreach, ongoing case management and individualized service plans to residents. This position is responsible for providing a range of supportive interventions to increase stability for residents, focusing especially on housing retention and behavioral health. As part of ongoing support, the case manager coordinates with other members of the team by referring residents to other needed services (ie., medical, legal, etc.).

Job Conditions
This position requires the employee to work in an environment where there may be exposure to blood, bodily fluids, and other potentially infectious material. Other exposures could include cleaning supplies, chemicals involved in pest control, paint and other materials used in building maintenance. Working conditions may include interruptions, interactions with angry persons, and exposure to computer CRTs.
