Qualifications:
- Some prior experience conducting investigations in the disabilities field preferred
- Associate Degree in Human Services or related field required with the minimum of two years’ experience working with individuals with intellectual or developmental disabilities
- Excellent verbal & written communication skills; Strong time management skills & ability to meet/exceed deadlines
- Working knowledge of OPWDD & HCBS regulations, guidelines, procedures & philosophy
- Proficiency in Microsoft Office programs (Word, Excel, Outlook, etc)
- Valid driver’s license & personal vehicle to use during workhours required
Responsibilities:
- Conducts incident investigations; ability to make decisions & determine outcomes, as well as preparing summaries with written recommendations to the appropriate departments
- Reviews and assures that incident reports are submitted to OPWDD and the NYS Justice Center, in a timely manner
- Provides timely and effective incident management communication and updates via OPWDD’s Incident Reporting Management database (IRMA) for all New York State regional offices.
- Collects and coordinates incident management data to identify trends. Contributes to the annual trend report by providing analysis of all incident management activities and statistics
- Site visits to all agency program locations to assist with relationship building
- May conduct orientation and other trainings related to incident reporting and incident management, as well as the NYS Justice Center and OPWDD, for various levels of staff and newly hired employees, as assigned
- Ability to assist with monitoring 24/7 hotline calls (on rotating basis) and escalate matters of importance appropriately
- Prioritizes job tasks to ensure timely and accurate completion of required work. May be assigned other tasks and duties reasonably related to job responsibilities.
