STATUS: Full-Time

LOCATION: 330 Monroe Avenue

DEPARTMENT: Home Care

SCHEDULE: Monday-Friday 8a-4pm (rotation of admin on call may be necessary depending on dept needs)

ATTRIBUTES

  • Master’s Degree in Nursing (MSN), Nurse Practitioner (NP), Healthcare Administration, or related field preferred. 
  • Proven track record of managing clinical operations and achieving measurable outcomes.
  • Familiarity with electronic health record systems and quality improvement methodologies.
  • Strong leadership, communication, and interpersonal skills.
  • Analytical and problem-solving abilities with a focus on patient care excellence.
  • Capacity to balance clinical priorities with financial and operational considerations.
  • Proven ability to deliver sustained levels of high patient satisfaction. 
  • Experience working in highly matrixed organizations. 
  • Experience in integrating complex clinical operations and distinct cultures. 
  • Demonstrated record of accomplishment of redefining and creating new models of care focused on quality outcomes for the communities served. 
  • Results-driven leader with experience in developing, designing and executing clinical programs that create measurable outcomes in quality, service, finance, growth and people.
  • Experience with population health and value based care deliver.
  • In-depth understanding of home care and hospice regulations, standards, and best practices or related experience.

EDUCATION:

BS: Nursing (Required), MS: Healthcare Business Administration, MS: Nursing

LICENSES / CERTIFICATIONS: 

BLS - Basic Life Support - American Heart Association (AHA), NP - Nurse Practitioner Certification - New York State Education Department (NYSED)

PHYSICAL REQUIREMENTS:

M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

$90,000.00 - $130,000.00

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
