Job Description

Assignment Level III

Under general supervision, with moderate latitude for independent judgment and decision making, in

addition to performing the duties of the lower Assignment Levels at a higher level either supervises the

duties of social work staff or performs tasks in a specialized clinical and/or programmatic area. Typical

tasks include:

Examples of Typical Tasks:

1. May provide direct clinical supervision and guidance to social work staff, screen and assign cases,

schedule staff, read and review staff work, evaluate work performance and/or maintain time and leave

records.

2. As a Licensed Clinical Social Worker (LCSW), provides and documents clinical supervision of NYC

H+H Licensed Master Social Workers (LMSWs) toward advanced Social Work licensure in alignment

with the all relevant professional standards and practices, and in accordance with all applicable NYS

licensure laws.

3. Maintains individualized supervision logs documenting clinical supervision and ensures timely

completion of attestation forms required by NYSED Office of the Professions to support LCSW

licensure applications, as applicable.

4. May conduct or lead special studies and projects including the collection and evaluation of data related

to vulnerable patient populations and prepare and present reports related to these service populations.

5. Establishes and maintains professional social work standards of performance concerning interview

techniques and methods and practices used in preparing clinical assessments, developing treatment

plans, and in documenting case histories

6. Interprets clinical information based on case data, histories, statistics, etc.; prepares and submits

comprehensive reports of case findings including chronologies of medical data and social factors

necessary to support recommended courses of action.

7. Orients and educates members of other professional disciplines by developing training curricula and

presenting on social work concepts and functions.

8. Trains and instructs staff in agency policies and procedures and in appropriate social work techniques,

methods and practices.

9. Ensures compliance with accepted social work practices and principles and operational adherence to

government rules and regulations, including compliance with reporting requirements.

10. Participates in conferences, committees and meetings on- and off-site with supervisory/managerial

staff, other clinical professionals and with representatives of government and other agencies and/or

community organizations.

11. Analyzes population health issues relevant to the area of responsibility and serves as a liaison to

community agencies and/or groups.

12. Liaises with community agencies, facilitating communication between the agencies and the

department/health care setting and the utilization of available community resources.

13. In the temporary absence of the supervisor, may perform the duties of that position.

Direct Service:

1.Screens all newly registered prenatal patients through the prenatal high-risk assessment tool

2.Assesses patient’s need for service and documents findings in the medical record.

3.Completes a psychosocial assessment on all patients at risk and develops a treatment plan.

4.Provides services including supportive, educational, individual, family and group counseling, planning for the care of the infant, referrals for concrete services and advocacy as needed.

5.Refers patients at-risk for HIV infection to the HIV Testing and Counseling Program and provides services for all patients confirmed as HIV+, documenting on the required case management form. Opens cases and follows-up on all HIV+ patients.

6.Provides crisis intervention, individual and group services as indicated.

7.Provides back-up social work coverage in the Maternity Inpatient; may provide backup in other areas as needed.

8.Provides services as assigned in related areas in the absence of Social Work staff.

9.Shares with other disciplines, problems identified in the clinic for appropriate action and follow-up.

10.Attends monthly prenatal administrative meetings and other relevant meetings as indicated.

11.Provides Social Work consultation as needed.

Administrative:

1.Prepare reports/statistics as needed.

2.Reports to supervisors any policy or procedure problems affecting the delivery of services to patients.

3.Attend monthly staff meetings and special programs as they occur.

4.May represent the unit/department at community meetings and hospital programs as assigned.

Minimum Qualifications

For Assignment to:

Level I

Limited Permit issued by the New York State Education Department (NYSED) in Social Work. Employees working with a Limited Permit must obtain a valid Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) within one (1) year of the date of appointment.

Level II

1. Valid license as an LMSW issued by the NYSED; or 2. Valid license as an LCSW issued by the NYSED.

Level III

1. Valid license as an LCSW issued by the NYSED.
